In the

section, after explaining the details of the procedure to the patient,select the relevant options.
Note:
To document additional details on the consent, click the
Consent
link.
To record that you answered all the questions asked by the patient, select the
Questions answered
check box.
To record that you explained each procedure and the associated risk to the patient, select the
Procedures/risks were explained
check box.
To record that you confirmed the side and exact location of the procedure, select the
Correct side and site
check box.
To record that you confirmed the identity of the patient and that it is the correct patient, select the
Correct patient identified
.
To record the patient's position during the procedure, click
Position
and select the details.
Note:
To record a position not available in the list, select the blank field at the top and type in the position.
To select the site preparation method used for the procedure, click
Prep
and select the details.

Document the Cosmetic or Therapeutic Injection Procedure
The cosmetic injectable template presents an enhanced female image to enable you to record the specific injectable, dosage, and location of each injection. You can document multiple injections, dosage, and location of each injection. On the GET template, click Cosmetic Injectable.The Cosmetic Injectable template opens. In the Procedure section, do one of the following: For cosmetic injections, select Cosmetic. For therapeutic injections, select Therapeutic. When you select Therapeutic injections, the Service Item field is automatically filled with the appropriate service item ID. To add a diagnosis, click Dx1 Description, search for and then select an appropriate diagnosis from the Diagnosis Search window. Note: To add the second diagnosis, click the Dx2 Description field. Locate and select the correct diagnosis. Click Status and select a status from the resulting list. Note: Your system may be configured to add an entry in the Status field. Click the Service Item field and select the

Document the Injectable Used for the Procedure
On the GET template, click Cosmetic Injectable. The Cosmetic Injectable template opens. In the Procedures section, type the name of the injectable in the Injectable field. To add the treatment number, click Treatment # and type the number. Type the manufacturer's lot number from the packaging for this injectable in Mfr Lot #. To select the manufacturer's expiration date as listed on the packaging for this injectable, click Expiration Date. The Expiration Date calendar window opens. Select the date and click OK. The selected date appears on the Expiration Date field. To select the date the injectable was mixed if using an injectable mixed at your practice, click Mix Date and use the calendar. The Mix Date calendar window opens. Select the date and click OK. The selected date appears on the Mix Date field. Complete the Cosmetic or Therapeutic Injectable Documentation
On the GET template, click Cosmetic Injectable. The Cosmetic Injectable template opens. In the Procedure section, type the total amount to be billed in the Total Billed field. Note: The amount you enter may differ from the total calculated injected if you are discounting the treatments for a patient or if the selected service item is configured to bill by a different unit. For example, if the service item code is configured for vials, you would accurately indicate the injection amounts in units at each site on the image, however, you would enter the total number of vials used in the Total Billed field. If a follow-up visit is required, click Follow up and select the appropriate time frame to schedule a follow-up visit. If a follow-up is required on need basis, select either the If symptoms persist or PRN check boxes or both as appropriate. Document a Cosmetic Injection
On the Home Page - Dermatologytemplate, review the patient information entered by the intake staff. On the Intake template, complete the Reason for Visit panel providing complete details for each reason of visit. On the Histories template, review the patient information and make any modifications if needed. In the Review of Systems panel, click Derm - ROS. On the Dermatology -ROS template, document the required information. To access the Graphical Exam Tool template, click GET. Click the Cosmetic Injectable link.The Cosmetic Injectable template opens. Note: The Cosmetic Injectable template can also be accessed from the SOAP template. In the Pre-Procedure care section, document the required information. Cosmetic Injectable Template
Using the Cosmetic Injectable quick link you can access the Cosmetic Injectable template and record cosmetic injectables such as a filler or cosmetic Botox or a therapeutic injectable such as Botox for migraine treatment or facial pain management. You can record the specific injectable, the dosage, and the location of each injection and add it to the grid. The Cosmetic Injectable template enables you to document multiple injectable types during a single session. You can also document and submit charges for injectable procedures directly from the template. Document Generic Procedure
On the SOAP template, go to the bottom of the template and select the Procedures icon. The Udp Procedure Picklist opens. Click Generic.The Generic Procedure template opens. In the Pre-Procedure Care section, after explaining the details of the procedure to the patient select the relevant options.To add more information about the consent, select the Consent link. In the Procedure section, do the following: To add a procedure description, click Procedure description, and select the details. To add additional procedure details, select the required information in the Additional Codes field. To add descriptions, click Description, and from the resulting Diagnosis window, select the appropriate description and code.
