Document Pediatric Symptom Checklist

You can identify cognitive, emotional, and behavioral problems in children aged four and above.

1. Open NextGen® Enterprise EHR.
2. Open the Pediatric SOAP template.
3. In the Screening panel, click the Child Development History button.
   The Child Dev. Rec template opens.
4. Click the Detail link in the PSC section.
   The Pediatrics Symptom Checklist (PSC) template opens.
5. Enter the details and select the appropriate option for each screening question.
6. To generate score, click Calculate.
   

7. Click Save & Close.
   The PSC score appears in the Screening Tool Results grid. The color of the Details link changes to indicate the information is documented and the Last done field displays the date of the survey.
8. To view the PSC data, select the row in the Screening Tool Results grid, and then click the View Only button.
