Inactive
In the
Summary of progress
section, select one of the following options:
No progress
Minimal progress
Some progress
Good progress
Enter comments in the associated field.
Click
Add
to move the data to the grid.
To modify a grid entry, highlight it to populate the fields above with the data, make the desired changes and click
Update
.

Document Assessed Needs, Goals, Objectives, and Interventions
Any previously entered data fills the grid under the Assessed needs section on the panel. On the Assessed Needs, Goals, Objectives and Interventions Overview panel, click the Display status of field and select a value to sort the grid. Click the Add button beneath the grid to enter an Assessed need on the template. In the Priority field, click to type a number. Type an explanation in the Assessed Need field. In the Status field, select a value from the Need status to display list. Enter a date in the Identified field. If applicable, set the Resolved date. If the need has been declined, deferred, or referred out, enter a Rationale. Click Save. Do one of the following: Click Clear for Add to enter other assessed needs. Click Close to return to the IAP tab. Start an IAP Revision/Review
To access the BH - IAP Revision/Review template do one of the following: Click the IAP Review tab. Click the IAP Review link in the navigation bar. Click the IAP Review link available from most of the Behavioral Health tabs in the template set. On the BH - IAP Revision/Review template, click the Program name field to select a value. Program names can be added and/or edited by the practice on the Custom Picklists Setup, system template. Set the Review date. Select Review or Revision. Enter a future date for Next review due by. Add a New Goal for IAP Reviews
When entering data in the Goals and Objectives Status Section panel, it is important to note that needs and interventions are not mandatory to complete the plan, but goals are required. The grid at the bottom of this panel offers a quick view of the client’s current needs, goals, and objectives. In the Goals and Objectives Status Section panel, select the New Goal link to open the BH- New Goal template. BH-New Goal Select a previously entered need for Linked to assessed need and click OK. In the Functional area field, select from the list. Type a new Goal and enter the Start date and Target completion date. Note: If the start and target dates are not entered before selecting an Adjusted target date, the system alerts the user to enter them. Enter the Desired outcomes for assessed need in individual's words. Record the Supports, resources, organizations and individuals needed to meet this goal. Record Transition/Discharge Criteria
In the Transition/Discharge Criteria panel, if appropriate, place a check mark next to No change. Enter the expected duration of treatment in Estimated length of treatment and stay. Enter details for How will provider/individual/guardian know level of care changes are warranted. In the Discharge plan field, type the anticipated plan for discharge, including support services. In the Development Plan Participation section, answer No or Yes to the statement: Others participated in the development of this plan. If Yes is chosen, enter a value in the field labeled: If yes, list names. Select No or Yes for Individual has participated in the development of this plan. Create a New Intervention
To enter a new intervention on the Intervention panel, click New Intervention.The BH - New Intervention template opens. Click the Linked to goal field to select a goal from the list and click OK. In the Linked to objective field, select an item from the list of previously entered objectives. Type an Intervention in the associated field. Click the Service description/modality field to select a value from the Modality list or to use the top, blank line in the list to type a different modality. In the Frequency field, do one of the following: Select a value from the list. Close the list to type a new kind of frequency. Set Responsible (type of provider) to a value available from the list or close the list to enter a different type of provider. Select Active or Inactive. The BH - IAP Review/Revision Template
The BH - IAP Revision/Review template is used to document progress or lack thereof and demonstrates evidence and/or rationale for the amendment of treatment goals and objectives in the Individual Action Plan (IAP). The system automatically pulls forward data from the last IAP or IAP review to the current encounter.
