Patient's Social History

You can document a patient's social history. You can open the individual social history template or the Adult Social History template to document the social history information of a patient. The Adult Social History template includes the existing social history templates arranged in a one-page panel format. Each panel includes the data fields from the respective social history template. You can choose to document the social history information of the patient on individual social history templates or on the Adult Social History template. You can document the following social information for an adult patient on the social history templates or the Adult Social History template: Tobacco/Vaping Use Alcohol/Caffeine Statuses Lifestyle Occupation Comment Diet History Environmental Note: NextGen® Adaptive Content Engine is packaged with the individual social history templates as the default social history templates. Pre-Configuration of MA Intake in the System Template

The MA Intake sub-navigational link needs to be configured in the Framework Content system template. To add the Family Planning Intake in the Framework Content template, complete the process below. After the configuration is complete, the MA Intake sub-navigational link displays in the Histories template. Open the Framework Content template. In the Specialty field, select Gynecology. In the Tabs section, select Office Visit in the Visit Type field. In the grid, scroll down and highlight Office Visit-GYN. Framework Content template In the Sub navigation section, highlight the Histories row. In the Sub Nav 5 section, type "MA Intake" in the Caption field. Overview of the Histories Template

You can add, update, delete, and review a patient's history summary details. You can document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history on the Histories template. Patient's Problem List You can manage a patient's problems. Patient's Medical History The Medical/Surgical/Interim panel allows you to document the medical or surgical history of the patient. Patient's Social History You can document a patient's social history. Patient's Family History You can document the family history of the patient in the Family panel on the Histories template. Patient's Diagnostic Tests and Results You can document a patient's diagnostic tests and the associated results. Document Pregnancy History

Select a field for the type of pregnancy. Select an amount from the numeric keypad. Click the Social History - Tobacco Use link and enter the applicable information. Click the Additional Screening link to enter the additional visit information in the Additional Visit Information Config practice template. Click the Adolescent Counseling link. Click the Parental Involvement Encouraged checkbox. Note: The Adolescent Counseling and Parental Involvement Encouraged fields may not appear for all patients. Click Save & Close. Document Intimate Partner Violence Screening

In the Intimate Partner Violence Screening section of the Family Planning Intake template, you can document intimate partner violence screening. Select a date for the Date of last intimate partner violence screening field. Select the applicable checkbox(es). If any selections are made, then additional questions appear. Select the Yes or No option for the Have you ever felt pressured to have sexual activities or get pregnant when you didn't want to field. Select Yes or No option for the Has anyone ever threatened or physically hurt you field. If you select Yes, select the applicable option for the Was a report filed field and select a date in the Date report filed field. Document Reproductive Health

On the Family Planning Intake template, in the Reproductive Health section, select the patient's preferred pronoun. The Preferred name field displays the patient's preferred name as recorded in Patient Demographics of NextGen® Enterprise PM. Enter a current birth control method. For Reason for no birth control (at Intake), select a reason. Note: The Reason for no birth control (at Intake) field appears only if the Title X Fields feature is enabled on Practice Configuration. In the Desired Birth Control Method field, select Same as current or Other. If you select Other, select a different birth control method from the additional textbox that appears. For Unprotected intercourse within last 5 days field, select Yes or No. For Unprotected intercourse since LMP field, select Yes or No. To indicate the importance of preventing pregnancy for the next year, select a number from a scale of one to ten.
