Add Additional Detail to an Entry

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. On the Functional Status Summary tab, expand the Functional Status Summary panel. Right-click the corresponding entry and select Open.The Care Management Note template opens. Type additional notes in the Details text box. Select Save, then Close, or use the arrows to advance to the next row/last row. Parent topic: Functional Status Tab

Functional Status Tab

You can view a summary of all current care management orders in place for the patient. The template consists of a single panel, the Functional Status Summary panel, that displays a summary of the five categories being tracked for the patient. You can also update existing information the Functional Status tab provides. Create a DME Order

Outside DME (Durable Medical Equipment) can be documented on the Pt Dme template. The Pt DME template can also be launched from the Outside DME Order link in the navigation bar. Open NextGen® Enterprise EHR. On the Objective tab, navigate to the Documents and Printing panel. Click the DME button to open the Pt DME template. Pt Dme Highlight an assessment in the grid located in the Encounter Diagnoses section to associate with the order. Note: An alert message appears if you attempt to enter a DME type without previously selecting an assessment. Select medical equipment by category of Assistive devices, Supplies, and Splints. Click the Assistive Devices field and select from the picklist.The service item CPT code and ICD code appear in their respective fields automatically. Document Transitioning into Care from Any Specialty

You can document transitioning into care from any specialty affected by transition into Care Management. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select a Specialty that is affected by Transitioning into Care Management and select Visit Type as Transition into Care. In the General Information panel, select the Transitioning Into Care link.The Transitioning Into Care template opens. Transitioning Into Care On the Transition into Care Management panel, select the Discharged field, and select the discharged date from the calendar. Select the Initial Communication field, and select the appropriate option from the list of communication types. Document Functional and Cognitive Assessments

You can document a patient's functional and cognitive assessments in the Functional and Cognitive Assessment Status panel. The data entered in the Functional and Cognitive Assessment Status panel is shared with the Health Promotion Plan, Medicare Preventive HPI, Screening Summary, and Care Plan templates to measure compliance and for surveillance reporting. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link. The Health Promotion Plan template opens. Expand the Functional and Cognitive Assessment Status panel. Functional and Cognitive Assessment Status For Functional Status and Cognitive Status, do the following: If there is no change in the functional status of the patient, select No Change. Document Referrals for Durable Medical Equipment

You can document a referral to Durable Medical Equipment (DME) for a patient in the Referrals Order template. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. Open the Intake template, select the Family Practice specialty and Office Visit visit type, and then select My Plan.The My Plan window opens. Select the Referrals tab. The Referrals Order template opens. You can select the information icon to learn more about the Referrals Order template. On the Referrals tab, select the insurance provider. Enter the insurance policy number, and then select the referral type. In the To section, select DME. In Specialty/Therapy/DME, select a service item from the list.
