Dynamic HPI Findings for a Reason

The Dynamic HPI Config practice template enables you to build different findings based on the associated sections for each reason of visit in the Findings section. Findings The Selection Type and Parameter field determine how the findings are presented on the dynamic HPI. The documented value in the Finding Type field determines how the sentence appears in the dynamic HPI template: The mutually exclusive item displays the selected finding as a separate sentence within that section. The multichoice single item enables the user to select multiple findings to be concatenated into a single sentence structure. The multichoice multiple item enables the user to select multiple findings so that each finding has its own sentence structure. In the Chief Complaint fields, select from the following three options: side location symptom A detailed explanation of the options for the side, location and symptom fields are given in the Chief Complaint Definition Options section of this guide. Dynamic HPI Config Practice Template

The Dynamic HPI Config practice template enables you to configure elements for the dynamic History of Present Illness (HPI) templates. Each dynamic HPI template is linked to a specific reason for visit. The dynamic HPI templates that are packaged with NextGen® Adaptive Content Engine can be modified to meet the specific needs of a practice. If there is a need for a custom reason for visit, the practice can create a custom template. Data sets for labs, diagnostics, and procedures can be configured in the Section and Finding sections. New sections such as family history, lab data, and diagnostic data can be configured for a reason for visit. The new sections appear in the dynamic HPI template in the left pane under the Reason field in run time. The Contradiction setup link enables you to create guidelines that prevent users from selecting clinically contradictory findings. Pediatrics Dynamic Parameters

The dynamic HPI templates for the Pediatrics subspecialties have both generic and distinct sections available for documentation. Generic sections are common to all reasons for visit and distinct sections are specific to each reason. A combination of these sections allow for the complete documentation of the patient's reason for visit on the current encounter. The following table defines the generic sections that appear for all Pediatric subspecialties. Note: An asterisk (*) indicates that the section includes a free text an entry option to enter the appropriate data if the desired option is not available. Parameter Description Intro Includes the introduction and the purpose of the patient visit. You can select either initial visit or follow up visit type for the chief complaint. Onset Includes options to select the onset date or period, and the age of onset of the chief complaint. Status Includes a list of options to indicate the status of the chief complaint. Link Chief Complaint to the Dynamic HPI Template

On the *Configuration Home system template, click Manage Template Links in the Configuration Navigation pane. On the Manage Custom Links template, click Link to and select Chief complaint to HPI pop up to launch from the list. Note: If you select the Show all custom links in the Link to field, all the filters are cleared. Enter the reason that you have created in the Chief Complaint field. Click the Popup to field. Select the hpi_dyn_right_left_unilat option to create a dynamic HPI and then click OK. Select the Gender field and select a gender to which the custom link is applicable. Click the Age field and select from the all, adult, or peds options to designate which age group the custom link is applicable to. Click the Specialty field to select the applicable specialty, and then click OK. Note: It is recommended that a single entry be created for an HPI and tied to a specialty. For example, the GI bleeding HPI is only made available for the Gastroenterology specialty and not in the O

Open the Intake template and enter a Specialty and Visit Type.Enter Physical Therapy for the Specialty and Physical therapy evaluation for the Visit Type. In the Reason for Visit panel on the Objective tab, do one of the following to open the corresponding HPI template: If no Reason for Visit was entered during the intake process, select a reason for visit value (for example, elbow) . If a Reason for Visit appears in the grid, click the desired row. In other specialties, the Reason for Visit panel is also available on the SOAP tab. At the top of the dynamic HPI template, a ribbon field appears with any other documented reasons. To change the Reason on the dynamic HPI template, do one of the following: Click the blue arrow next to the Reason field to select a value from the list. In the top ribbon, click the arrow to toggle and select a previously documented reason and move that value to the Reason field. Click intro in the left pane. Each reason includes an intro.
