You can configure care teams from the Care Team Configuration template, in Practice Team Configuration, and Practice Care Team and Risk Score Review panels.
To configure a care team, do the following:
- Create practice care teams
- Add members to the team and assign primary providers
- Update and remove members from the practice care team
- Filter the care team and member details based on the team name and member status
- View patients and their risk scores associated with teams
- Generate Practice Care Team and Risk Score Review reports
