BH - Individual Counsel/Psych Prog Note Template

The BH - Individual Counsel/Psych Prog Note template enables you to document individual counseling services and the client's progress or the lack thereof. Any risk assessment, goals, objectives, or assessment data that was entered previously on the Comprehensive Assessment template set populates in the panels found on the Progress Note tab. You can document new issues and make updates to the goals, objectives and interventions. Use the Behavioral health billing section to submit billable service codes to the Superbill. The Progress Note tab can be a stand-alone template or used in conjunction with the other tabs or links. The BH - Psychopharm template is intended for the Behavioral Health prescriber to document a follow-up visit. The interventions can be tied to the client's treatment plan, but the content is specific to the needs of psychiatric professionals who prescribe medication. Complete the General Panel of the BH Psychiatric Evaluation

To access the BH - Psychiatric Evaluation do one of the following: Click the Psychiatry Eval tab. Click Psychiatric Evaluation from the navigation bar. Navigate to the General panel and click the Program name field. Program names can be added and/or edited by the practice on the Custom Picklists Setup, system template. Select the appropriate name from the list. Click the clock button to enter the current time in the Start field. The Start and End buttons are available here and at the bottom of the Psychiatric Eval tab in the Behavioral health billing section. The values documented in either place are immediately reflected in the same fields in the other section. Entered times can be modified by clicking in the field. Document the Patient's Initial Psychiatric Evaluation Summary

As a psychiatrist, nurse practitioner (NPP), or other behavioral health professional, you can document an initial psychiatric assessment of the patient. It is important to note that you can prescribe medications during the psychiatric evaluation. Also, the EM Coding button opens the Finalize template, which enables you to submit billing codes. Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Psych Eval (BH), and then select OK. Note: You can open the Psych Eval (BH) template from your Packet Navigation, or from the Templates module. Enter the Prescriber's Evaluation

Expand the Prescriber's Evaluation panel. If there are no modifications, select the No changes reported/observed checkbox. In the Interim Update field, use the My Phrases link or manually type information about the client's progress since the last visit related to symptoms, effectiveness of medications, substance use, changes in family/social history, overall functioning, and any new issues.
