Dynamic HPI Templates
In the NextGen® Adaptive Content Engine templates, the following types of History of Present Illness (HPI) templates are used to document a patient's reason for visiting the office: Static HPI - displays hard-coded content that is not easily customizable; a single template is available for each complaint with options for selecting pre-determined details: the final concatenation is available when you close the template Dynamic HPI - enables you to add, edit, update, and delete dynamic content for the HPI and concatenates the content on the template as items are selected or edited. NextGen® Adaptive Content Engine is packaged with pre-configured dynamic HPI templates for the Gastroenterology, Physical Therapy, Orthopedics, Urology, and Pediatrics Well Child. Access Well Child Dynamic HPI
In the Intake template, select the Specialty as Pediatrics and Visit Type as Well Child. Click the Pediatric SOAP tab. In the Reason for Visit panel, select well child hpi from the left pane. Reason for Visit on the SOAP Template
Open the SOAP template. Do one of the following: If the Introduction field contains text, review the content and make any necessary changes. If the Introduction field is blank, compose an introduction for the patient. The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If checked, clear the Do not Launch HPI check box. To add additional details to a previously entered reason for visit, click one of the reasons for visit in the data grid.The appropriate HPI template opens. To add a new reason for visit: Select a complaint from the reasons for visit in the left pane. In the Other field select a reason from the resulting list or type a reason for visit.The associated HPI template opens. On the HPI template, enter the appropriate data. Enter any comments. Document with a Non-Laterality View
Open the Intake template and enter a Specialty and Visit Type, for example, Gastroenterology and Office Visit. Click the SOAP tab, expand the Reason for Visit panel, and select GERD. The dynamic HPI appears with the selected item in the Reason field. To access the other documented reasons in the Reason for Visit panel, click the right or left arrows in the top ribbon. Click Intro in the left pane and select a finding to populate the concatenation field. An asterisk indicates that the selected item has been added to the concatenation. The selected data also fills in the History of Present Illness column on the shared grid that appears on both the HPI and SOAP templates. Click another section in the left pane to continue building the dynamic HPI based on the available findings. The left pane can include free text an entry as an option. Document the Patient's Reason for Visit as a Provider
The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close. Reason for Visit for Well Child
You can use the Reason for Visit panel on the Pediatric SOAP template to review and update the reasons for visit added during the intake process. The Well child HPI appears on the grid automatically if you select Well child for Visit Type. Reason for Visit In addition to documenting the specialty specific reason for visit available in the left pane, you can also document a well child visit on the a generic HPI template by selecting Well child or Well child HPI in the Reason for Visit panel. In the Reason for Visit panel, you can: Review and update the social history of the patient using the Pediatric Social History link. Open the Interval History- Nutrition template using the Nutrition/Elimination link. Document substance use, psychiatric, placement, sexual practices, and child abuse history of the patient using the Confidential History link. Document the Well Child Reason on the Generic HPI Template You can document the reason for well child visit on the generic HPI template. Document
