Document Discharge Summary

You can document the discharge summary and close the case of a participant who is leaving or closing out from the Child Welfare program. On the CW Summary/Case Closure template, select the Program field, and select the program that the participant is getting discharged or closed. Enter the discharge date and select the Close Program check box. Expand the Discharge Summary Case/Closure panel. Discharge Summary Case/Closure To update the initial licensure date and withdrawal date, enter the dates in the fields. Optional: Enter the initial adoption certification date. Indicate if the family has adopted the child. If you select Yes, do the following: Enter the pre-adoption name, date of birth, and adoption date of the child. Select Add. The new record appears in the grid. You can update and remove the adoption details of an adopted child before closing the discharge summary case. Child Welfare Summary and Case Closure

You can document the details when a parent (main participant) leaves the child welfare program. When a parent (main participant) leaves a foster care program, the providers document the discharge summary on the CW Summary/Case Closure (BH) template. The foster parent can leave the program for a variety of reasons. Parent adopted their foster child. Parent are no longer open to fostering or adoption. Parent are not eligible for foster care programs. CW Summary/Case closure Panel You can use the Signature (Tasking Supervisor/Staff for review and signoff) panel on the template to request secondary approval or attestation from another user. You can send a task to the approver requesting approval. The approver can use this panel to add their electronic signature and generate a Signature Attestation document that is separate from the template document. Open the CW Demographics (BH) Template

You can document the demographic details of children and family members on the CW Demographics (BH) template. In NextGen® Enterprise EHR, select a patient, and open an encounter. On the Categories tab, select Open Template. The Select Template window opens. Select All. In the Demographics tab, select CW Demographics (BH). The CW Demographics (BH) template opens. Open CW Home Interview (BH) Template

You can conduct an in-depth interview of prospective foster parents and document the details using the CW Home Interview (BH) template. In NextGen® Enterprise EHR, select a patient, and open an encounter. On the Encounters tab, select Open Template. The Select Template window opens. On the Medical Records tab, under Show, select All. The list of all medical record templates appears. Select CW Home Interview (BH), and then select OK. The CW Home Interview (BH) template opens. You can document the initial screening details of prospective foster parents on the CW Phone Interview template. In NextGen® Enterprise EHR, select a patient, and open an encounter. Document Child Welfare Demographics

You can document relevant demographic information such as participant information, address and contact information, placement preferences, and family household information. Open NextGen® Enterprise EHR. Open the CW Demographics (BH) template. Optional: Select the Special Assistance checkbox, and select the appropriate option. In each panel, document the required information. Select Save & Close.
