Document an Adolescent's Self-reporting Trauma Assessment

Open NextGen® Enterprise EHR. For a patient encounter, open a template. Note: Your practice can also configure the template to be accessible from the Packet Navigation (BH) template. For more information, go to NextGen Healthcare Success Community, and download the Configuration Guide forNextGen® Behavioral Health Suite. Select the Screening Tools link to open the Screening Tools template. Select CATS Self-Report (7-17 Years) to open the template. Note: The template appears in the template list only when the patient's age is between 7-17 years. Select the assessment date. In the Traumatic Events panel, document the stressful or scary events thats the child encountered. In the Symptoms and Functioning panel, rate the questions that the child has faced in the last two weeks. Note: The Symptoms and Functioning panel is active only when you have answered at least one question in the Traumatic Events panel. Select Save & Close. Documenting Trauma Assessments for Children and Adolescents

The Child and Adolescent Trauma Screen (CATS) is an assessment tool aligned with DSM-5 criteria, designed to evaluate potentially traumatic events and post-traumatic stress symptoms (PTSS). The CATS can be administered either as a self-report or through an interview and is suitable for preschoolers, children, and adolescents. There are specific self-reported versions for children and youth aged 7-17, as well as there are also two caregiver-reported versions: one tailored for children aged 3-6 and another for those aged 7-17. The version for younger children adheres to the DSM-5 PTSD symptom criteria for the 3-6 age group. An interview format may be more appropriate for younger children or for youth who face challenges with reading comprehension. Document the Final Assessment Details on the Patient's Suicide Status

Open NextGen® Enterprise EHR. For a patient encounter, open a template. Note: Your practice can also configure the template to be accessible from the Packet Navigation (BH) template. For more information, go to NextGen Healthcare Success Community, and download the Configuration Guide forNextGen® Behavioral Health Suite. Select the Screening Tools link. The Screening Tools template opens. Select CAMS Suicide Status Form (SSF-5) Final Session. Select the assessment date. In the Patient panel, let the patient complete the details with your assistance. In the Clinician panel, document the patient's suicidal details. In the Clinician Post-Session Evaluation panel, document your evaluation of the patient's assessment details. In the Signature panel, document the necessary signatories. Select Save & Close. Document the Patient's Therapeutic Assessment Notes

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select CAMS Therapeutic Worksheet (BH), and then select OK. Note: You can open the CAMS Therapeutic Worksheet (BH) template from your Packet Navigation, or from the Templates module. For more information on configuring Packet Navigation, go to NextGen Healthcare Success Community, and download the Configuration Guide for NextGen® Behavioral Health Suite. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later. Enter the therapy session number for the current therapy. Enter the patient's suicide details, such as personal story behind the suicide attempt, direct and indirect drivers influencing the suicide attempt, and the risks impacting the severity of the suicid

Complete the Patient's Needs and Strengths Assessment

You can document the needs and strengths of adult and child patients on the ANSA and CANS assessment templates. Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select one of the following templates: Adult Needs & Strengths Assessment (BH): to open the assessment template for adults. CANS, birth to 5 (BH): to open the assessment template for children up to the age of five. CANS 5-17 (BH): to open the assessment template for children between the ages of 5 and 17. Select OK.The selected assessment template opens. Note: As the CANS and ANSA templates included in the NextGen® Behavioral Health Suite are state-specific, an application message is displayed while opening the templates.
