Document a Clinical Opiate Withdrawal Scale

Open the Detoxification Monitoring (BH) template, and then expand the COWS and CIWA Assessments panel.
To start a new order, select Start New Detoxification Monitoring.

If completing an assessment for a current order, you must not Start New Detoxification Monitoring.

The initial score is cleared for a new treatment series.
Select the COWS link.
The Clinical Opiate Withdrawal Scale (COWS) window opens. By default, opiate detoxification appears in Reason for this assessment.
Answer all the questions.
The system calculates the Total score. The Performed Date, Hr, Min, and Performed by default to the current date, time, and user logged in.

Note: All questions must be answered for a total score to be calculated. If all questions are not answered, the template saves the answers, but the total score is not calculated. An alert appears to confirm if you want to proceed without a total score.
To modify the Performed Date, Hr, or Min, select the respective field and make the modification.
Select Save.
To document multiple COWS assessments for the current order, select Clear for Add, and then repeat steps 3-5.
Select Close.
The information appears in the COWS grid.
To generate the assessment document, select Generate Document.

Document the Patient's Detoxification Status

You can obtain information regarding how to use the template or sections of the template by selecting the information icon or the How to Use This Template link. Open NextGen® Enterprise EHR, and then in Patient History, select Templates. The Select Template window opens. Select Demographics, and then select All. Select Detoxification Monitoring (BH), and then select OK. Note: You can open the Detoxification Monitoring (BH) template from your Packet Navigation, or from the Templates module. Detoxification Monitoring

With this template, you can: View detox orders. Document the patient's vital signs. Complete a standard CIWA and/or COWS assessment. Calculate the next assessment date and time. Complete the order when the treatment plan is. Detoxification Monitoring (BH) Template The CIWA-Ar and COWS grids display all the assessments that have been completed for the patient. The Current assessment and Assessment history options enable you to filter the grid. The CIWA total score and COWS total score fields display the patient's most recent assessment score while the initial scores are displayed in the respective fields. Complete the Diagnoses Section for Transfer/Discharge

Document the Patient's Progress in the Opiate Treatment Program

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Opiate Treat Progress Note (BH), and then select OK. In the Time Entry Navigation sidebar, select Billing Time Entry, and then enter the start time. Note: You can select the current time or enter the time manually. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can document your impressions and comments, differential diagnoses, and patient or provider plan details for an assessment using the A/P Details tab.
