From the Treatment Recommendations/Assessed Needs panel, in the Further evaluations needed section, select the evaluations needed. Select None indicated if no other evaluations are needed. Document the Level of care/indicated service recommendations. Enter a value for Individual served/guardian/family response to recommendations. Enter Treatment Recommendations and Assessed Needs
In the Treatment Recommendations/Assessed Needs panel, select the priority by selecting Priority. Enter the Assessed need. Select Status to select from the list. Enter date Identified. Enter the Rationale. Select Add. The information is added to the grid. Enter Treatment Recommendations
The Treatment Recommendations/Assessed Needs panel enables the delineation of assessed needs, their priority, their status, the date identified, the rationale for not addressing a particular need, and the date resolved. The priority, assessed need and status are required fields if this section is used. If no recommendations are indicated, select No additional recommendations clinically indicated. To enter an assessed need, click the Priority field to type a number. Free text an explanation in the Assessed Need field. In the Status field, select a value from the Need status list. Enter a date in the Identified field. If the need has been declined, deferred, or referred out, enter a Rationale. If the assessed need is resolved, enter the Resolved date. Enter other assessed needs if required. To manage existing assessments, highlight a row in the grid and click Update or Remove. To delete data from the related fields before it is saved to the grid, click Clear. Select any Further evaluatio

Document Treatment Recommendations/Assessed Needs
On the Treatment Recommendations/Assessed Needs panel, if appropriate, select No additional recommendations clinically indicated. To create a new entry, click the Priority field to enter a number. Type an Assessed Need. In the Status field, click to select a value from the Need status list. Enter a date in the Identified field. If applicable, enter Rationale(s) and/or a Resolved date. Click Add. Repeat steps 1 - 6 as needed. In the Further Evaluations Needed section, do one of the following: Select None indicated. Place a check mark next to any of the applicable options. Enter details in the Level of care/indicated service recommendations field. Type a summary in the Individual served/guardian/family response to recommendations field. In the Treatment Planning Updates section, select No or Yes to Change in IAP Required. If Yes is selected, complete the IAP revision by clicking IAP Review at the top of the template. Edit a Treating Assessment
On the Subjective template, General panel, Treating Assessments section, select the assessment in the grid to be updated. Click Edit to launch the PT Treating Assessment template. Select the item to be updated in the Treating assessment grid. Make any updates or changes as needed. Click Update and then click Save & Close. Parent topic: Treating Assessments
