Enter Family Information
- On the Family Information panel, select No or Yes for Does the patient have children?
If Yes, enter demographic information about the client’s children:
- First and Last name - required fields
- Relationship - to the client; required field
- Date of birth
- Age - automatically displays when the Date of birth is entered; this value updates based on the birth and encounter dates
- Living with
- Identify custody issues
- To remove the information entered in the fields before it has been saved in the grid, select Clear.
- Select Add.
The information displays in the grid. - To modify any information in the grid, do one of the following:
- To modify an entry, select a row, make the changes, and select Update.
- To remove an entry from today's encounter record, select a row in the grid and select Remove.
- In the Family history of mental illness and/or substance abuse (who and what relationship to client) section, select Add to access the Family Health History template. For information on the Family Health History template, go to NextGen Healthcare Success Community and download the Getting Started User Guide for NextGen® Adaptive Content Engine.
- To indicate no family history, select No relevant family history or Adopted - no family history known checkbox.
- To Include Family Mental Hx on Document, select the respective checkbox.
- In the Family history and relationships field, enter up to 500 characters in the text box.
- Pertinent family medical, mental health and substance use history including Adult Child of Alcoholic (ACOA)/Child of Substance Abuser (COSA) status fields, enter free text or use My Phrase to document relevant information.

- You can use the Comments field to enter any additional information.
- You can select the Family and Social Support or Behavior Management link to enter additional Function Needs information.
