Add an Existing Diagnosis to the Current Encounter

  1. Open NextGen® Enterprise EHR.
  2. For a patient encounter, open an assessment template.
  3. From the Assessment/Diagnosis panel, select a diagnosis in the Active Diagnosis grid.
    The Diagnostic description and ICD Code field fills in.
  4. Alternatively, in the Last Encounter Diagnosis for Current Provider section, select Copy Forward.
  5. Add Status, Severity, Differential Dx, and Impression.
  6. To enter specifiers in the Specifiers field, do one of the following:
    • Select in the field and enter free text.
    • Select the My Phrases link and select a phrase from the list.
    • In the Present on Admission field, select a description from the list.
    The Description field fills in based on the selection made.
  7. Select admitting diagnosis and select whether the diagnosis was present on admission, with the description.
  8. To mark the diagnosis as chronic, add the diagnosis to the client’s clinical problems list, or add the assessment to my tracked problems, select the respective checkbox.
  9. To add the diagnosis to your favorites list, select the My favorites checkbox and then select the arrow Drop-Down Icon to select the list you want to add it to. You can create a list by selecting in the field and entering text. The text entered becomes the list name.
  10. Select Add To Grid.
    The information displays in the grid.
  11. Select Close.
    The Assessment/Diagnosis panel displays your selected diagnosis in the grid.
  12. To add Psychosocial Stressors or Referrals, select the respective link available on the screen.
  13. To remove an assessment, select the assessment in the grid and select Remove.
