Add Diagnostic Details

In the Assessment/Diagnosis panel, confirm the assessment selected displays in the Description details and ICD Code field. If not, select the assessment in the grid to fill in the details. Add Status, Severity, Differential Dx, and Impression. To enter specifiers in the Specifiers field, do one of the following: Select in the field and enter free text. Select the My Phrases link and select a phrase from the list. If applicable, select Present on Admission and then select a description from the list. To indicate the diagnosis is chronic, add the diagnosis to the client’s clinical problems list, and/or add the assessment to my tracked problems, select the respective checkbox. Select Add To Grid. The information displays in the grid. To add psychosocial stressors, select the Psychosocial Stressors link. To add referrals, select the Referrals link. To reorder the diagnoses, select Sort. Add a Diagnosis Using Common Assessment

From the Assessment/Diagnosis panel, select the Add Common Assessment link. The Common assessment window opens. Select a diagnosis. If the diagnosis you selected has a + in front of it, more than one code exists. You need to select the link and select from a secondary list. The BH – Severity window opens. Select the severity from the list and then select Close. To add additional diagnoses, repeat steps 2 and 3. Select Save & Close to save your assessment and return to the previous screen. Behavioral Health Assessment and Diagnosis

You can link a diagnosis to the current encounter and enter assessment details from the Assessment/Diagnosis panel. Practices that use institutional billing must provide an admitting diagnosis. You can enter the diagnosis in the Admitting Diagnosis field. If your practice does not use institutional billing, this field is not required and can be left blank. You can use the following links on the panel to search for a diagnosis code: Active Diagnosis allows you to use a diagnosis from a previous encounter for the client. Diagnosis Code Lookup allows you to add a diagnosis to the client’s medical chart. This should only be completed by staff members authorized to assess and diagnosis a client. Common Assessment allows you to select from a limited set of diagnoses to add to the client’s encounter. This should only be completed by staff members authorized to assess and diagnosis a client. Add Diagnosis Codes and Spacers to an Assessment Page

You can add spacers and diagnosis codes with their descriptions to an assessment page. On the Specialty Common Assessment Config template, expand the Manage Common Assessments panel. Select the page to be updated from the *Common assessment page name field. The name of the selected page appears in the *Common assessment page name field, and the items available in the selected page appear in the assessment item section. Select the item number, where you want to add the item. The selected item number is filled in the Item # field. Select Description and code or Spacer from the Item to add field. The results are as follows: If you select Description and code, the Description field with the Diagnosis Code Lookup link appears, and the Code and Full description fields appear. Patient's Problem List

You can launch the diagnosis search window from a Problem List Pop-Up trigger to display only the diagnoses and ICD codes that have been assigned to the patient. You can then select an ICD code from the list. For example, you may want to associate an existing ICD code with a procedure for billing purposes. Note: In this mode, you cannot search All Diagnoses, Favorites, or Categories; use ICD Legacy Mapping; or view, add, or select ICD synonyms. Enter your search criterion in the text box at the top of the diagnosis search window. You can enter all or part of the clinical description or ICD code or description. Select Search. The search results grid displays the patient problems that match your search criterion. To close the diagnosis search window without making a selection, select Cancel. To select an ICD code, double-click the result row you want, or select the row and select Select. The ICD code is automatically entered in the template field assigned that value. Search All Diagnosis

Enter your search criterion in the text box at the top of the diagnosis search window. You can enter all or part of the clinical description, ICD code, or billing description. You can search for both ICD-9 and ICD-10 codes. You can enter a higher-level ICD code to display all of the associated lower-level ICD codes. Note: The search criterion may be populated from the trigger. You can change it, if necessary. If you search for ICD code 250.00, the search results include codes such as 250.01 or 250.02. Select All Diagnoses (default) in the left pane. Select Search, or select the Enter key. The search results grid displays the results that match your search criterion. Any results that match a diagnosis already on the patient’s diagnosis list display at the top followed by any results that match the provider’s most frequently assigned list.
