The following table describes the column name and description that can be used while making column selections.

Column	Description
Policy Nbr	Displays the policy number.
Insured	Displays the insured person's name.
Card	Displays card image.
Detail-2	Displays additional information if available.
Benefit Info	Displays benefits information.
Elig	Displays eligibility request information if available.
Note	Displays note.
