Add Patient Information to a Patient Chart

You can update patient demographics, contact, and relationship details. A relationship refers to any individual who is connected to a patient and using the patient's insurance to cover charges. Relationships are enterprise-wide. When a relationship is created in a different practice, the relationship automatically appears when you create a chart in your practice.

  1. Open NextGen® Enterprise PM.
  2. On the main toolbar, select Chart, and then in Patient Lookup, enter your search criteria, and then select Find.
    Matching people and patient records appear in the search results.
  3. In Matching Records, select a patient record , and then select Open.
    The Patient Information tab opens in the Patient Chart window.
  4. On Patient Information, select the open folder icon next to Name.
    The Demographics tab opens in the Modify Patient Information window.
  5. Enter the patient address, contact, and demographic details.
    The details are updated on the Demographics tab in Patient Chart.
  6. Optional: If the patient has expired, select the Expired checkbox and expiration date.
    The patient expiration message appears on the Patient Information tab in Patient Chart.
  7. Select the Relationship/Role tab, enter a relationship, contact, guarantor, head of household, and support role, and then select OK.
    You can update, delete, and hide a relationship. However, you cannot delete a relationship if the person is the guarantor on one or more encounters for the patient in any practice.
    The relationship details are updated on the Relationship/Contact/Guar tab in Patient Chart.
