Patient Chart Demographics with Providers Report

The Patient Chart Demographics with Providers report pulls patient demographics information and user-defined patient provider information from the Provider tab of the Add/Modify Patient Information window.

User Defined Patient Providers are created in Practice Preferences > Provider. Up to 12 provider preference names can be defined. Once created, the Provider tab becomes available on the Add/Modify Patient Information window. Multiple providers can be recorded for a patient, each with a Preference Name, Provider Name, Effective Date, Expiration Date, and Expiration Reason.

To access the report in NextGen® Enterprise PM, select the Reports menu and select General and then select Patient Chart Demographics with Providers.
