Field	Description
Height (In Inches)	Enter the patient’s height at the time of the encounter.
Weight (In Pounds)	Enter the patient’s weight at the time of the encounter.
Temperature	Enter the patient’s temperature at the time of the encounter.
Pulse	Enter the patient’s pulse at the time of the encounter.
Blood Pressure	Enter the patient’s systolic and diastolic pressure at the time of the encounter.
User Defined Fields: Clinical UD 1-4	Up to four additional encounter-level custom fields can be created using the User Defined Fields in the Encounters preferences under Practice Preferences. Enter a caption or label for one or more of the Clinical UD 1-4 fields. Users can then make numeric entries in these custom fields on an encounter.

Encounter Maintenance - Encounter Specifics Tab

The Encounter Specifics tab displays when one or both of the following are enabled in Practice Preferences > Encounter: Encounter Specific User Defined Fields CCBHC Qualifying Encounter The Encounter Specifics tab includes the following fields: Field Description User Defined Fields: Encounter Specific 1–50 Up to fifty additional encounter-level custom fields can be created in User Defined Fields in the Encounters preferences under Practice Preferences. Enter a caption or label for one or more of the Encounter Specific 1-50 fields. Users can then make free text entries in these custom fields on an encounter. CCBHC Qualifying Encounter Select this check box to indicate the encounter includes a Certified Community Behavioral Health Clinic (CCBHC) service and should be flagged as an encounter that qualifies for CCBHC reporting purposes. If the encounter has a charge for a SIM code that is flagged as a CCBHC Qualifying Encounter code in the Service Items Library, then this check box is auto

General Information about Patient Encounter

The General tab on the Encounter Maintenance window includes high-level information about the patient's encounter, which is used for general reporting purposes and, specifically, for billing and claim purposes. Field Name Description Patient Type Displays the types of patients available in the master list on the Master Lists window in File Maintenance. Condition Related to Employment Indicates that the encounter of the patient is related to the patient's employment. This option populates box 10a on the CMS 1500 paper claim form. Remarks Displays additional information related to the patient encounter. Same/Similar Date Displays the date when the patient first experienced the same or similar symptoms. This option populates box 15 on the CMS 1500 paper claim forms. Complaints Displays the text complaints for the patient encounter, up to 40 alpha-numeric characters. Additional Information about an Encounter

The Properties tab displays additional information about the encounter, but the fields that display this information cannot be edited. The Properties tab includes the following read-only information: Field Description Checkin Displays the date and time the encounter was created using the Checkin/Create Encounter process for individual appointment and group appointment in the Appointment Book. Checkout Displays the date and time the encounter was checked out using the Checkout process for individual appointment and group appointment in the Appointment Book. Elapsed Time Displays the amount of time elapsed between the Checkin/Create Encounter and Checkout processes for individual appointment and group appointment. Encounter Date Displays the original date and time for the encounter. This information will not change even if the Billable Date and Billable Time on the encounter are changed. Locked By If the encounter is locked in NextGen® Enterprise EHR, this displays how the encounter was

Field Descriptions in Encounters Practice Preference

The following table describes the fields in the Encounter practice preference. Field Description When creating a new encounter, set types to You can select the encounter types that should be selected by default in the Encounter Types field on the Create Encounter window. The Create Encounter and Encounter Maintenance windows include specific information related to an individual patient encounter. The window includes the following tabs: General Clinical Billing & Collections Encounter Specifics UB Claims Marketing Properties History The following fields are available at the top of the Create Encounter and Encounter Maintenance windows: Field Description Billable Date This is the date of the encounter. If the encounter is created from an appointment, the Billable Date defaults to the appointment date. Otherwise, the date defaults to the current system date and can be modified if needed. Billable Time This is the billable time for an encounter. The billable time is set to 12:00 AM by default. The billable time changes to the current system time if you select the Default Enc Billable Time to System Time check box in the Encounters preference on the Practice Preferences window. Set Up an Encounter for Regular UB Users

Access the Encounter Maintenance window for the patient and encounter the UB claim is being created for if you want occurrence and value codes to display on the UB claim. In NextGen® Enterprise PM, select the Chart icon on the main toolbar. The Patient Lookup window appears. Enter the search criteria and then select Find. To open the chart, right-click the chart and then select Open. On the Encounters tab, right-click the encounter and then select open menu icon . The Encounter Maintenance window appears. Select the UB tab, then the Occurrence Codes sub-tab. Enter the occurrence codes and dates in the field locator fields, if applicable. To enter the value codes and associated dollar value or unit value, select the Value Codes tab and enter the data in the Field Locator fields. Select OK.
