Add Demographics Information
Demographic information includes details such as marital status, mother's maiden name, student status, preferred language, religion, primary care provider, and primary dental care provider. Veteran and smoker statuses can also be noted.
You can enroll the patient in NextGen® Enterprise EHR the Enterprise Chart feature to view the patient’s encounters in every practice in your enterprise.
- On the Add Person Information or Add Patient Information window, select the Demographics tab.
- From Marital Status, select one of the options:
Note: Marital Status is a required field in the Add Patient Information window and appears in red, if left blank.
- Annulled
- Domestic Partner
- Legally Separated
- Married
- Single
- Widowed
- Divorced
- Interlocutory
- Life Partner
- Polygamous
- Unknown
- Enter the maiden name of the patient's or person's mother.
- To identify a patient or person as a student, select the status in the Student Status field. Select one of the following:
- <none>
- Not a Student
- Part Time Student
- To identify the race, ethnicity, and preferred language of the patient, do the following:
Note: The Race and Ethnicity fields may not appear on this tab. Depending on how the application is set up, these fields may appear on the UDS tab.
- From Race, select one or more races that the patient identifies with.
Select select here to Select Race Order to set the order in which the races appear in the field. If your patients are primarily Hispanic, you can set Hispanic as the first entry in the list.
- From Ethnicity, select one or more ethnic groups.
Select Click here to Select Ethnicity Order to set the order in which the ethnic groups appear in the field. If your patients are primarily Central American, you can set for Central American as the first entry in the list.
- Select the patient's preferred language.
- From Race, select one or more races that the patient identifies with.
- To add a patient's preferred correspondence language, select the Corr Lang link.
The Correspondence Language window opens.
- Select the correspondence language and select OK.
Note: Refer Correspondence Language for more informationA message appears stating that the correspondence language is updated and prompting you to verify the Preferred Language.
- To enter the patient's religious profile, do the following:
- Select the patient's religion.
- Enter the name of the church that the patient attends.
- To enter the patient's Native American tribal identity, enter the tribal identity code in Community Cd.
- To enter the patient's primary care provider and dental care provider, do the following:
Note: If you enter the provider's name into the field, the provider's name is not added to the Providers master file.
- Enter the name of the patient's primary care provider.
- Enter the name of the dental care provider.
- To track the patient as a veteran or a smoker, select the check box.
- To enroll the patient in Enterprise Chart, select Enterprise Chart.
- Select OK.
- Correspondence Language
You can save the preferred correspondence language for printing or previewing letter and statements.
