Add a Support Role
- Open the Person Information or Patient Information window for the desired person or patient.
- Select the Relations/Roles tab.
- In the Support Role section, select the Open Menu
button, and select New.
The Add Support Role Information window appears. - In the Last field, enter the person's last name. This field is required.
- In the First field, enter the person's first name. This field is required.
- In the Middle field, enter the person's middle name.
- In the Birth Date field, enter the person's birth date.
- In the Billing Address section, enter the person's address with the following information:
- Street Line 1
- Street Line 2
- City
- State
- Zip
- Address Type
Note: Items available for selection are from the Address Type master list in File Maintenance. - In the Relationship field, select the person's relationship to the patient. This field is required.
Note: Items available for selection are from the Relationships code table in File Maintenance.
- In the Support Role field, select one of the following. This field is required.
- Caregiver
- Emergency Contact
- Next of Kin
Note: Items available for selection are from the Support Role Type code table in File Maintenance. - In the Telephone Number section, enter the following numbers:
- Home and Comment
- Work, Extension and Comment
- Cell and Comment
- To save changes and close the window, select OK.
Note: If the new support role has the same Last Name and First Name as an existing support role, the following prompt appears: Potential duplicate support role records have been found. To continue entry of the current person select OK. To update one of the existing records select Cancel, then re-open the existing support role record and edit the appropriate field.
