Button	Description
	The Save Form/Label Template button saves your changes to the form.
	The Delete button removes the selected data field from the form.
	The Insert Data Field button opens the NextGen Data Repository so that data fields can be added to the form.
	The Font button enables you to set the font type for free text entered in the form.
	The Template Properties button enables you to set margins and paper size for the form.
	The Template Sorting Preference button enables you to specify the fields you want the forms to be ordered by as they print in NextGen® Enterprise PM. For example, the forms could be sorted by the last name field as they print.
	The Exit Form Design button closes the report. The File Maintenance desktop displays.

Insert Data Fields in Form and Label Templates

When you design forms and labels, you can use the NextGen Data Repository window to select and place the data fields that you want displayed when you print your custom designed forms or labels. To insert data fields in a form or label template: Open the template in File Maintenance. In the Form Design window, select Insert Data Field . The NextGen Data Repository window opens. Double click on the applicable icon to display the data fields for that icon. Insert a data field in one of two ways: Select and drag the data field onto the Form Design window and release the mouse button when you have positioned the data field where you want it placed. If you want to adjust its position on the form, select and drag it again. Select an item in the list, and then select Add. Use any of the following options to design the fields: To select more than one field, select the Shift key while you select the fields. To align multiple fields, select the fields. Create Forms

Form templates enable you to create templates that can then be used for printing the practices' forms. Using form templates allows you to customize the forms to best meet your needs. A form template can only be one page in length. You cannot rename a form template. Open File Maintenance. Select System Master Files, then PM, and then Forms. The Forms List window opens. Do one of the following: To add a new item, select open menu , and then select New. To modify an existing item, select the item in the list, select open menu , and then select Open. You can also double-click to open the item. The Form Design window opens. Select Insert Data Field on the form toolbar. The NextGen Data Repository window opens. Insert or modify the data fields. If necessary, modify the template properties. Select Save on the form toolbar. The Save Form Template window opens. Enter the template name, and then select OK. In the Form Template Name field, enter a name for the template, and then select OK. Create

Form and Label Templates Printing Sort Order

You can specify the form fields that you want to use to sort a form or label template based on the form fields used to create the form and label template. When you set up the form fields that you want to use to sort the forms or labels by, the form or label templates automatically display in that sort order when previewing or printing in batch mode. Note: You can select a maximum of five form fields for the printing sort order criteria. If you attempt to select more than five form fields, a window displays to remind you. Forms

You can create and modify templates to standardize patient forms that are used by your practice. The form templates determine the structure for patient forms, such as where certain information must be placed on the form. For example, if you have pre-printed fee tickets, you can set up a template to your specifications. You can create label templates for mailing labels, chart labels, form labels, or any other labels you need. To design your own forms, you need to access the Forms Templates master file from File Maintenance > System > Forms master file. Add a Patient's Picture to a Form or Label

The application enables you to add a patient's picture to a form or label through Form Templates and Label Templates. After you add the picture to the template, you can print the forms and labels with the picture in NextGen® Enterprise PM. Open File Maintenance. Select System Master Files, then PM, and then either Forms or Label template. The Forms List or Label Templates List window opens. Select the template to which you want to add the picture. Select Open Menu , then select Open. If you want to create a new template, see Creating Forms or Creating Label Templates. The Form Design or Label Design window opens. Select Insert Data Field on the form/label toolbar. The NextGen Data Repository window opens. Double-click Patient Information. A list of patient information fields displays beneath it. Select the Patient Picture data field and drag the item from the list and drop it on the template in the location where you want the picture to print. The data field is inserted in the template

Forms and Labels

Forms and Labels can be created in File Maintenance and then printed for an individual patient, or for multiple patients in NextGen® Enterprise PM. Any type of form that is used by a practice can be created in the Forms master file, or the Forms Enhanced master file. Some forms may be used to gather information from a patient, such as an intake form. Other forms may be used to provide information to a patient or to their insurance provider. Each form is created using a combination of free-text verbiage for static information on the form, and data fields that are automatically populated with patient-specific information when the form is printed. Labels that are used by a practice can be created in the Label Templates master file. Examples include mailing labels, chart labels and specimen labels.
