Form templates enable you to create templates that can then be used for printing the practices' forms. Using form templates allows you to customize the forms to best meet your needs. A form template can only be one page in length. You cannot rename a form template. Open File Maintenance. Select System Master Files, then PM, and then Forms. The Forms List window opens. Do one of the following: To add a new item, select open menu , and then select New. To modify an existing item, select the item in the list, select open menu , and then select Open. You can also double-click to open the item. The Form Design window opens. Select Insert Data Field on the form toolbar. The NextGen Data Repository window opens. Insert or modify the data fields. If necessary, modify the template properties. Select Save on the form toolbar. The Save Form Template window opens. Enter the template name, and then select OK. In the Form Template Name field, enter a name for the template, and then select OK. CreateCreate Forms

The application enables you to add a patient's picture to a form or label through Form Templates and Label Templates. After you add the picture to the template, you can print the forms and labels with the picture in NextGen® Enterprise PM. Open File Maintenance. Select System Master Files, then PM, and then either Forms or Label template. The Forms List or Label Templates List window opens. Select the template to which you want to add the picture. Select Open Menu , then select Open. If you want to create a new template, see Creating Forms or Creating Label Templates. The Form Design or Label Design window opens. Select Insert Data Field on the form/label toolbar. The NextGen Data Repository window opens. Double-click Patient Information. A list of patient information fields displays beneath it. Select the Patient Picture data field and drag the item from the list and drop it on the template in the location where you want the picture to print. Before you create an enhanced form, you must have a least one form group created. To create a form, do the following: Access File Maintenance. Select System Master Files, then PM, and then Forms Enhanced. The Forms Enhanced window appears. In the Form Group field, select a form group. Note: You can select (All) Forms, but you will be prompted to select a form group upon saving. Right-click on a form group from the list that is displayed and select New. Select Blank Form. A blank form displays, allowing you to create the body of the form by entering and formatting text, inserting pictures, and selecting data repository fields as needed. Select Save after creating your form. In the New Form Template window, select a Form Group Name (if you did not do so previously), Form Name and Form Description. Note: You can only enter up to 40 characters for the Form Name and Form Description. Select OK. Notice the new form group in the list displayed. You can create and modify templates to standardize patient forms that are used by your practice. The form templates determine the structure for patient forms, such as where certain information must be placed on the form. For example, if you have pre-printed fee tickets, you can set up a template to your specifications. You can create label templates for mailing labels, chart labels, form labels, or any other labels you need. To design your own forms, you need to access the Forms Templates master file from File Maintenance > System > Forms master file. Open NextGen® Enterprise PM. Open a patient chart, and then select the Encounters tab. Right-click an encounter, and then select Print > Forms. The Print Forms window opens. In Template, select the form template that you have set up as a separator sheet. In Master File, select Document Types. Select the document type master file you want to associate with the System Master File Text data field on the form template. When you scan the separator sheet with barcodes, the File menu in NextGen® Document Management displays the selected document type and the fields associated with it. Note: The document type that you assign when printing the form template must have an MRN field that is set for automatic search. Select Print. Note: To use the barcode engine, activate it in the User Preferences window and then use the Scan & File feature. After creating a custom DB picklist in the Custom DB Picklists system template, you can add triggers to a text field on any template using Template Editor. This enables you to open the newly created custom DB picklist to select a value for the field. In Template Editor , create a copy of the template to be modified and save the new copy with a unique name, as detailed below: Open Template Editor from the Application Launcher. From the File menu, click Open. The Open window appears. In the Template name field, enter the template name to be modified. Click Open. The template opens. From the File menu, click Copy Template. The Copy Template opens. In the Template name field, enter a unique name for the template. The format for naming the file is XXX_Filename, where XXX is the system's abbreviation to identify the template. Click Make Shared. Click Copy. A copy of the template opens in Template Editor. Select the desired field. Right-click and select Triggers. Review and modify the trigger
