Add Payer-Specific Facility IDs

Open a location master file. Do one of the following: To create facility IDs at the system level, select the Location Defaults tab. To create facility IDs at the practice level, select the Practice tab, and then select the Use practice level claims data check box. Note: Facility IDs on the Practice tab override the ones on the Location Defaults tab. Select Payer Specific subtab to create payer-specific facility IDs. On the Payer Specific subtab, right-click in a row and then select New. To modify an existing ID, right-click on the row and then select Open. The Add/Modify Payer Facility ID window displays. Select the payer name, and then enter the facility ID. Do not use a specific payer name more than once. Note: If the Use practice level claims data check box is selected on the Practice tab, the payer-specific facility ID on the Practice tab overrides any claim-type specific IDs. If no pertinent facility exists on the Practice tab and the check box is selected, the facility for HCFA 1

Add Facility IDs to Locations

You can define claim type-specific or payer-specific facility IDs for a location at either the system or practice level. Blue Cross Blue Shield, Medicaid, and Medicare use the claim type facility IDs. Some payers of the same claim type (as specified in the Payer master file) might assign a different facility ID to a location. You can create payer-specific facility IDs for a location. Location Information - Practice Tab

The Practice tab on the Location Information window contains practice-specific claims and address information for the Location master file. If you enter information on this tab, it applies only to the practice that you are currently logged into and overrides similar information entered at the system level on the Location Defaults tab. Note: If you have the system level data set up and you also have a record for your practice data, the practice data will be assigned even if it is blank. This is especially important for the Facility Location check box. If the Facility Location check box and the HCFA Box 32 are not selected on the Practice tab and a record exists, then that location will not display on the Encounter Maintenance window for that practice regardless of the setting at the system level. Location Information - Practice Tab Field Descriptions

The following table provides descriptions of the fields located on the Practice tab of the Location Information window. Field Description Mammo Certification Enter the mammography certification number for the service location. This number displays in box 32 of the HCFA 1500. CLIA ID Enter the Clinical Laboratory Improvements Amendment ID for the service location. It is required at the Line item (FA0) level of the claim for each Medicare claim filing CLIA covered procedures. This will override the Authorization Number found in box 23 of the HCFA 1500. Each Lab can have a unique CLIA ID, relating to Service Location. If the CLIA number is included on non-CLIA procedures, then Medicare will ignore the CLIA ID. DME Provider Nbr Enter your DME provider number if each service location for the practice has a different DME provider number for DMERC billing. DME NPI Enter the same DME NPI for all DME regions. Locations Master File Report

The Locations master file report includes information from the Locations master file in File Maintenance. The report uses details from the Location Information window. Note: The Locations master file reports will be available in enhanced mode. Enhanced Reporting feature is added to improve system performance for specific reports. The reports are typically database intensive when generated due to the large amount of data reported. This feature is always enabled for Locations master file, you can utilize a new user interface and paging mechanism that allows you to generate, view, print, and export the reports in a timely manner. Any memorized reports that exist in the legacy form continue to display in legacy format. For better accessibility of reports, it is recommended to memorize the enhanced Locations master file reports. The report includes the following details. Set Up the Practice as an ASC in the Locations System Master File

The Ophthalmology ASC practice is required to be set up as an ASC Center in the Locations system master file. Before beginning to enter patient information for Ophthalmology ASC, you must select your ASC Center in the top toolbar as the Location. In File Maintenance, click Locations under Master Files - System to open the Locations List window.
