Practice Preferences for Patient Charts in Practices

Chart: User-Defined Names

The tables describe the preferences that you can set for patient charts.

Enter a label for each of the user-defined fields you want to display on the patient's chart.

To make a field mandatory for chart creation, select the corresponding Require checkbox. These user-defined fields represent information that remains constant from encounter to encounter, for example, "organ donor."

When the user-defined name is created and set as required, the information from the user-defined fields is displayed on the patient's chart, the Chart tab, and the Chart/Ins tab of the Appointment Book. You can also select the patient information fields in the data repository for form and label templates.

Chart Options

Required FieldsTo make a field mandatory, select the column to the right of the field..
Release of informationSelect the information that may be required by some payers and that is sent electronically. Release of information is the default for every encounter at that practice. The default is stored in the insurance record and displayed in the Patient Insurance - Encounter window.
Automatically assign benefitsSelect this option to automatically select Assignment of Benefits in the Patient Insurance - Encounter window from the Encounters tab of the patient chart.
Enable patient statusSelect to display the following:
  • The Patient Status tab in the Add/Modify People/Patient Information window.
  • The Patient Status section on the patient chart.

When a user selects the Patient Status and Pt Status Reason on the Patient Status tab, the information is also displayed in the Add/Modify People/Patient Information window.

Patient status required at Chart creationSelect this option to require the user to enter the patient status during chart creation.
Marketing plan required at Chart creationSelect this option to require the user to enter marketing information mandatory during chart creation. This option allows you to track how patients found out about your practice. You can create marketing plans in the practice master files.
Hidden marketing plans default to new EncountersSelect this option to allow marketing plans to default from chart details for all new encounters for a patient when the encounter is added directly from the chart.
Enable practice payer specific informationSelect this option to set up practice-specific co-pays. The co-pay is obtained from the Practice Level section of the Insurance Maintenance window. Otherwise, the co-pay is set as enterprise-wide.

Automatically reassign encounter balance responsibilitySelect this option to reassign the encounter balance to the next insurance (secondary or tertiary) or to the patient balance after the insurance is posted (that is, the line item status is Settled to [next payer]).

Default chart guarantor to self if patient is 18 or olderSelect this option if patients who are 18 years or older should be the default guarantor. This setting applies to Patient Chart > Demographics and to the Create Encounters windows.
Enable encounter payer copaySelect this option to display the Encounter Co-payment field and the Copay Exempt checkbox in the Insurance Maintenance window.

Birth Mother's full name on Relations TabSelect this option to make the Birth Mother’s Full Name field available in the Add/Modify Patient Information window on the Relations/Role tab.
Prompt for a reason when a Recall Plan is stopped
Select this option to do the following:
  • Require the user to enter a reason when stopping a recall plan.
  • Provide an option to stop active recalls in the Patient Information > Demographics window when the Expired checkbox is selected and an expired date is entered,
Appt Event SIM CarryoverSelect Default to encounter to automatically fill the fields in the Charge Posting window with the associated service items when it is initially accessed if an encounter is created for an appointment for an event.

You can enter the start date set for the SIM carryover.

Medical Record NumberSelect Req at Chart creation to require the user to assign the medical record number during chart creation. This number is displayed at the top of the patient chart in both NextGen® Enterprise PM and NextGen® Enterprise EHR. The default chart number is the next available number as determined by the counter master file. If this option is not selected, no medical record numbers are added to new charts.
If you select Req at Chart Creation, consider the following options:
  • Select Allow user override at Chart creation to allow the user to override the system-generated medical record number by entering a different chart number.
  • Select Allow preceding zeros to have zeros precede all medical record numbers for new and existing charts in the practice.
