Document a Screening with a Template Screening Tool
You can document information in a screening tool that opens as a template. Open the NTS Screening Tool template. In the Screening Tools Available list, select a screening tool that has a T indicator in the first column. Note: An asterisk (*) denotes a screening tool that has already been used in the current encounter. The corresponding screening tool template opens. Complete entries for the patient on the screening tool. If the template does not keep a running score, select the Calculate button to calculate the score. Save and close the screening tool. Note: The button to save and close the template may be labeled Save & Close, OK, or Accept. The screening tool name and results are saved in the Screening Tools Performed grid on the NTS Screening Tool template. Complete any other screening. On the NTS Screening Tool template, select Save & Close. The NTS Screening Tool template closes. The new screening results save to the Screening grid in the Plan panel of the Single Visit template. P

Document a Screening with Other Screening Tools
If you document information by using a screening tool that is not configured to open as a website or template, you can enter the results in NextGen® Therapy Suite. On paper or by some other means, record the results of the screening performed. Open the NTS Screening Tool template. In the Screening Tools Available list, select a screening tool that has no indicator (blank) in the first column. Screening tool names that have no entry in the first column do not open a template or a website. Note: An asterisk (*) denotes a screening tool that has already been used in the current encounter. The Screening Instrument field and the *Performed Date field automatically display the screening tool name and the encounter date. Manually enter the score, severity or interpretation, and any comments or screening plan. Select Add. The documented data appears in the Screening Tools Performed grid. Select Save & Close. Document a Screening with a Website-based Screening Tool
You can use a web screening tool to document patient screening information. The tool opens in a separate web browser window. Open the NTS Screening Tool template. In the Screening Tools Available list, select a screening tool that has a W indicator in the first column. Note: An asterisk (*) denotes a screening tool that has already been used in the current encounter. On the NTS Screening Tool template, the Screening Instrument field and the *Performed Date field automatically display the screening tool name and the encounter date. The screening tool website opens. Depending on your browser and security settings, you may see alert messages. Complete entries for the patient. Calculate the patient’s score. Close the screening tool window. In the NTS Screening Tool template, enter the score, severity and interpretation, and any comments. Select Add. Retrieve the FOTO Scores and Documents in the Encounter
Before you begin FOTO must be configured in the Single Visit section of the NTS Practice Config practice template. From the Single Visit template, expand the Plan panel. Get FOTO button on the Plan panel Select Get FOTO. The FOTO score and test name appear in the Screening Tool grid and the documents are attached to the encounter. Parent topic: Screening Grid

Types of Screening Tools
Regardless of specialty, all screening tools and configured by the practice are displayed in the Screening Tools Available section on the left. The entry in the first column indicates how the screening tool is made available. Template-based screening tools have a T in the first column of the Screening Tools Available list. When you complete and save template-based screening tools, the results are added to the Screening Tools Performed grid on the NTS Screening Tool template. Website-based screening tools have a W in the first column of the Screening Tools Available list. When you complete and close website-based screening tools, you must manually enter the score and interpretation on the NTS Screening Tool template, and then select Add to add the results to the Screening Tools Performed grid.
