Reading an HQM Patient Clinical Quality Report
The Patient Quality Reporting window displays the status by measure for the selected patient and reporting program and shows the total number of Treatment Opportunities. The patient's name appears on the title bar. The report groups measures for predefined reporting programs as configured within the client’s HQM portal. Each program appears in a separate tab at the top in alphabetical order. Select a tab to display the respective measures qualified for it. To sort information in a column in ascending or descending order, select the column heading. Note: For the CMS 179 measure parameters, a message appears for you to view the result in HQM. The report contains the following information: Program Title. The name of the program in the selected tab; for example, Registry (PQRS) Individual 2017. HQM Data Date (Information displayed is current as of mm/dd/yyyy). The date of the latest encounter data available to HQM. The date is defined by the schedule configured by the Client Administrator

Generate the HQM Patient Clinical Quality Report
Before you begin To generate HQM patient clinical quality report, following are the requirements: Select a patient who qualifies for HQM measure reporting data. Have the View Clinical Quality Reporting permission enabled in System Administrator operations rights. A non-provider user who is a designated delegate for a provider can view both the patient- and provider-level reports. Note: If these requirements are not met, the Top Toolbar and File menu options and the Patient Information Card for generating the report are disabled in NextGen® Enterprise. To have the latest data displayed in NextGen® Enterprise, a data push must be executed in HQM. In NextGen® Enterprise EHR, search for and open the chart for the patient to generate the report for. Do one of the following: From the File menu, select Clinical Quality Reporting and then Patient Level CQR. Select Patient CQR on the Top Toolbar. Generate the HQM Provider Clinical Quality Report
To generate this report, you must fit one of the following user profiles: HQM-enrolled provider who has data that qualifies for reporting. You can only view your own report. Your Provider Relationship must be Self in the System Administrator user settings. The practice you are logged on to must be enrolled in HQM Clinical Quality Reporting. Assigned delegate for a specific HQM-enrolled provider with qualifying data. You may view the report for the assigned provider only. User with the Administrator View permission enabled to view HQM Clinical Quality reports for all HQM-enrolled providers in the practice. This permission is set in System Administrator operations rights. If these requirements are not met, the Top toolbar and File menu options for generating Clinical Quality reports are disabled in NextGen® Enterprise.
