Document Individuals Present
In the Individuals Present panel, select from one of the Contact Type options. Select one of the following in the Individuals present section: Individual not present Individual present Select Others present and enter the details in the Support Resources section. In the Support Resources section, record the client's closest friends and relatives. To add the support resources data in the grid, click Add. To change the grid view, select All or Active. The Effective and End dates determine if a resource is Active. Record New Issues
Expand the New Issues panel. Under New issues/stressors/extraordinary events presented today, select from the available statuses. Enter relevant details in the Explanation field. Click the Task button to send an email to a coworker. BH - Contact Note Template
Use the BH - Contact Note template to document billable treatment contacts with the client. This includes goal sessions with Personalized Recovery Oriented Services (PROS), case management, residential programs, and Permanent Housing Programs (PHP). The BH - Contact Note template may be used, for example, by a case manager reviewing an Individualized Recovery Plan (IRP) with a client. This information connects to the standard objectives and interventions to assist in the overall care of the Behavioral Health client. These billable encounters must include the staff’s interventions and/or services provided and the individual’s response to those interventions.
