Document the Pregnancy History Panel
You can use the Pregnancy History panel to access the Pregnancy Outcomes template. Note: The Pregnancy Outcomes template is used during Prenatal and Postpartum visits as well as gynecology encounters. The workflow is different for each. In general, the most detail is entered during a Postpartum Visit. Pregnancy History Open the Histories tab. Open the Pregnancy History panel. Select the Add/Update button in the Pregnancy History panel. The Pregnancy Outcomes template opens. If you enable the Parity auto-calculation setting, the Auto-calculation green text appears. Auto-calculation text on Pregnancy Outcomes template If the Parity auto-calculation setting is disabled a red text displays. Document Pregnancy Details
You can document the pregnancy details for female patients as a part of medical history. On the Past Medical History template, in the Medical panel, click Add Pregnancy Detail. The Pregnancy Outcomes template opens. Pregnancy Outcomes template In the Quick Entry/Edit panel, in the Parity, Outcome Details, and Other sections, click the appropriate fields, and enter the values. In the Detailed Entry/Edit panel, enter the details of the pregnancy. Note: The fields that are marked with asterisk (*) are mandatory. Click Add To Grid to add the documented details to the Pregnancy history grid. Click Save & Close. Documenting Pregnancy Details On the Pregnancy Outcomes template, you can document the pregnancy details of a patient. Document Pregnancy Outcome in the OB Postpartum Template
Select the Postpartum tab. In the Pregnancy Outcome panel, select Add/Update. The Pregnancy Outcomes template opens. Enter the applicable information in the Pregnancy Outcomes template. The entered information displays in the Pregnancy Outcome grid. Verify the parity values are correct. Pregnancy Outcome grid Select the OB Problem List link. The OB Problem List template displays with the most recent pregnancy information in a new grid view. You cannot edit the information in the OB Problem List template. Select an entry in the Pregnancy Outcome grid. To generate the pregnancy outcome document , select the Pregnancy Outcome Document button. Document the Gravidity/Parity Panel
Before you begin You must enter the gravidity and parity information in the Pregnancy History panel. The Gravidity/Parity panel enables you to view the gravidity and parity information and enter multiple births. Open the Histories template. Expand the Gravidity/Parity panel. Note: You can only view the details in the Gravidity/Parity panel. Gravidity/Parity Optional: To enter multiple births, enter the number in the Multiple births window. Document Pregnancy Outcome
The postpartum template is used and completed by a provider. Select the Postpartum tab. Select Pregnancy Outcome link. The Pregnancy Outcomes template opens. Pregnancy Outcomes template Note: The nurse or medical assistant uses the link and the provider uses the panel to document instead. On the Pregnancy Outcome panel, you can highlight a line in the pregnancy grid and print out a summary of this pregnancy in a (newly-created) document. This would only be done when all of the information is entered for the particular pregnancy by the provider. Document the pregnancy entry type, pregnancy number, and termination type/type of delivery at a minimum. However, you can also select to enter the date, sex of the baby, whether full term or preterm, and complications. Select Add to Grid. The pregnancy outcome information is added to the pregnancy grid at the bottom of the template, enabling you to add additional babies, if necessary. Select Save & Close. Update an Edema Record
On the Objective template, navigate to the Edema panel. Highlight the row in the grid. Click Edit. The Edema details template opens. Make relevant edits on the template. Click Save. Parent topic: Document Edema Details
