Add a New Objective for the IAP Review

To enter a new objective in the Goals and Objectives Status Section panel, click New Objective. The BH - New Objective template opens. Select a previously documented goal from the list in the Linked to goal field. In the Objective field, choose from a list of objectives or type a new objective. The Objective field is required. Take notice of the counter for the Characters left to be sure that all of the data appears on the generated document. Set the Status for the Objective. Enter the Start date. Enter a Target completion date. If required, select an Adjusted target date. Set the IAP review date. Click Save & Close to return to the BH - IAP Revision/Review template. Add a New Goal for IAP Reviews

When entering data in the Goals and Objectives Status Section panel, it is important to note that needs and interventions are not mandatory to complete the plan, but goals are required. The grid at the bottom of this panel offers a quick view of the client’s current needs, goals, and objectives. In the Goals and Objectives Status Section panel, select the New Goal link to open the BH- New Goal template. BH-New Goal Select a previously entered need for Linked to assessed need and click OK. In the Functional area field, select from the list. Type a new Goal and enter the Start date and Target completion date. Note: If the start and target dates are not entered before selecting an Adjusted target date, the system alerts the user to enter them. Enter the Desired outcomes for assessed need in individual's words. Record the Supports, resources, organizations and individuals needed to meet this goal. Answer No, Yes, or Yes, but deferred to the question: Has a problem been identified and enter

Enter Treatment Plan Information

Some of the data in the Treatment Plan panel is shared with the Individual Action Plan and IAP Review templates. Updates must be made from IAP or IAP Review templates by clicking the respective links. Any new goals, objectives and interventions must be entered here. In the Functional area field, do one of the following: Click to select a value from the picklist. Click the top, blank row from the picklist to type an entry in the field. Making a selection from the Functional area picklist automatically opens picklists for you to choose goals and objectives. Click the Goal field to select a pre-existing value based on the functional area selection. This field is required before you can document data in the objective or intervention fields. If you attempt to enter the goal with an existing description, the following alert appears: A goal with this description already exists for this person. Enter Goals, Objectives and Interventions Addressed Today

Any goals and/or objective data that were previously entered on the Comprehensive Assessment template set populate in the panels found here. The grid on this panel offers a quick view of the client's current goals and objectives. In the Goals/Objectives Addressed As Per Individualized Action Plan section, field, click to Goal today select a previously entered item. Select an Objective today from the picklist of previously entered objectives. Click Add. In the Interventions Addressed Today section, do one or both of the following actions: Click Select From Previous IAP Intervention to select a previously entered value. Enter details in the Interventions/methods provided memo field. Click the My Phrases link to select a sentence to enter in the Interventions addressed today section. Click Add. Click Save & Close. Goals and/or objectives that were entered previously on the Comprehensive Assessment templates populate in this panel. The grid on this panel offers a quick view of the client's current goals and objectives. In the Goals/Objectives Addressed As Per Individualized Action Plan section, Goal today field, click to select a previously entered item. Select the Objective today from the list previously entered objectives. Click Add. In the Interventions Addressed Today section, click Select From Previous IAP Intervention to choose a previous entered value. Enter the Interventions/methods provided. Record the Response to Interventions/progress toward goals and objectives. Document the Issues referred to physician/psychiatrist for consideration. Document Assessed Needs, Goals, Objectives and Interventions

Any previously entered data fills the grid under the Assessed needs section on the panel. On the Assessed Needs, Goals, Objectives and Interventions Overview panel, click the Display status of field and select a value to sort the grid. Click the Add button beneath the grid to enter an Assessed need on the template. In the Priority field, click to type a number. Type an explanation in the Assessed Need field. In the Status field, select a value from the Need status to display list. Enter a date in the Identified field. If applicable, set the Resolved date. If the need has been declined, deferred, or referred out, enter a Rationale. Click Save. Do one of the following: Click Clear for Add to enter other assessed needs. Click Close to return to the IAP tab.
