Title X 2.0 Template Overview

The Title X 2.0 template displays all the Title X 2.0 clinical information recorded for a patient during a family planning encounter. You can review the present and previously recorded patient details for Family Planning Annual Report (FPAR) 2.0. The patient’s recorded Title X 2.0 information is categorized into various panels, such as General, Social History, Confidential History, Assessment / Plan Vital Signs, and Testing and Results. The Title X FPAR 2.0 report is required under the Title X Service Grants and is generated by all family planning clinics receiving the Title X grants. The Office of Population Affairs (OPA) requires all Title X grantees to collect and submit FPAR data at the encounter level for the yearly reporting period, starting January 1 through December 31. The Title X FPAR report 2.0 must be set up and licensed for your practice. When the Title X 2.0 software is installed, the lab tests and the results also appear on the Title X 2.0 template. When the Enable Title X Fields option is selected on the Practice Configuration template, providers can access the following Title X 2.0 fields for capturing information on the Title X Family Planning Annual Report (FPAR) 2.0 report. Configuring Title X Fields

The Title X report 2.0 is generated by all family planning clinics receiving Title X grants from the government. Enable Title X fields in practice template If you select the Enable Title X fields check box, additional fields appear on the Title X 2.0 content for the practices that need to adhere to the Title X FPAR 2.0 standards. For example, when you configure the Title X Fields setting on the Practice Configuration template, the Family planning encounter check box appears in the General panel on the Intake template and the birth control method information appears in the Assessment/Plan panel on the SOAP template. Update Patient Title X Information

OpenNextGen® Enterprise EHR. Select a patient and create an encounter. To open the Templates module on a patient encounter, do one of the following: On the Categories or Encounter History tab, select Open Template. On Top Toolbar, select Templates . On the main menu, select File > New > Template. The Select Template window opens. In the Show section, select All. Select Title X 2.0, and then click OK. The Title X 2.0 template opens. Title X Template Review the patient’s Title X 2.0 details. To qualify the encounter to be part of Title X / FPAR 2.0 report, you can select the Family planning encounter check box. Viewing Patient Assessment Plan

Title X Assessment Plan panel The Assessment / Plan panel displays patient's contraception methods at end of the visit based on the following clinical templates: Field Description Include all Title X contraception information in the document with patient permission This finding is documented in the Assessment / Plan panel of the SOAP template. Birth control method at end of visit This finding is documented in the Assessment / Plan panel of the SOAP template. Viewing Patient Confidential History

On the Title X 2.0 template, the Confidential History panel displays a patient's contraception and reproductive-life planning information based on the following clinical templates: Title X Confidential History panel Field Description Include all confidential information in the social history document with patient permission You can configure confidential information to be added to the social history document. Birth control method at beginning of visit Enables you to document the finding on the OBGYN Synopsis or Confidential Patient Information template. Reason no birth control method at beginning of visit Enables you to document the finding on the OBGYN Synopsis or Confidential Patient Information template. Would you like to be pregnant in the next year? Enables you to document the finding on the Confidential - Sexual Practices/STIs template.
