Orthopedic Home Page Template

The practice can customize the panels displayed on the Home Page - ORT template. The following table describes the panels on the template. Panel Description Reason for Visit Displays the reasons for a patient visit. Physical Exam Displays the physical exam details of the visit. You can view the system name, followed by abnormal exam findings, and the normal exam findings. Assessment/Plan Displays patient details related to assessments, follow-up details, treatment plans, lab orders, and office procedures. History Displays the patient history documented by the intake staff on the Histories template. Allergies Lists the allergies associated with the patient. Review of Summary of Allergies by Providers

A provider can start the visit by reviewing the patient's information documented during the previous patient encounters, on the Home Page template for the Allergy and Immunology specialty. The information documented by the nursing staff on the Intake template is displayed on the Home Page template for the provider's review. You can update the information on the Intake and SOAP templates. Reviewing Patient History on SOAP

You can review and edit a patient's medical history in the History panel on the SOAP template. The following categories in the History panel provide the corresponding history details. Past Medical/Surgical Interim Social Family Diagnostic History Panel The Social Determinants of Health and PRAPARE® links appear based on the Practice Configuration practice template settings. SOAP Template

You can review and modify the documented patient intake and histories information on the SOAP template. You can review the Asthma Flowsheet, Allergy Test Flowsheet, Spirometry, and Nebulizer panels. You also can document physical exam, assessment plan, and communication details. SOAP Link Icons The icons for various links appear on the SOAP template. Document the Patient's Reason for Visit as a Provider The reason for visit added by the intake staff on the Intake template appears on the SOAP template. Patient's History Summary Documentation by Provider You can document a patient's past medical or surgical history, interim, social, family, and diagnostic study history in the History Summary panel. Document Vital Signs The provider reviews the vital signs added by the Intake staff on the Intake template and makes the applicable changes. Reviewing Patient's General Information on SOAP

You can view the general details of a patient in the General panel on the SOAP template. In the General panel, you can do the following: Document the relationship of the person who provides the patient information, in the Historian field. Capture the electronic Summary of Care requests and receipts that are completed electronically, and provide the information as reportable fields on the Transitioning Into Care template. On the Addition Visit Information template, you can provide answers for the preconfigured questions in the Interpreter Details category. You must document interpreter details when someone else interprets the patient’s communication for you. Indicate if the patient for the current encounter is new or established. The provider reviews the information entered by the intake staff to determine if any additions or changes are required. The information documented by the nursing staff on the Intake template displays on the Home Page template for the provider's review. You can update the information on the Intake and SOAP templates.
