Physical Therapy Reasons for Visit

In the Physical Therapy template set, certain reasons for visit use the dynamic HPI template Reasons for Visit. The dynamic HPI view and the laterality or non-laterality view that appears is based on the particular History of Present Illness (HPI) parameter being documented. Physical Therapy HPI parameters that involve location for a chief complaint related to extremities (for example, pain or numbness related to a hip or knee) appear in the laterality view. Parameters that refer to the chief complaint overall, such as status or place of injury, appear in the non-laterality view. Reasons for visit that do not involve laterality, such as lumbar spine, use the non-laterality view for all HPI parameters.

Treatment visits can be based on the laterality or non-laterality view based on the selected reason for visit and the HPI element being documented.

You can document the following reasons for visit on the dynamic HPI template using the laterality view for certain parameters:

  • Ankle
  • Elbow
  • Foot
  • General orthopedic
  • Hand
  • Hip
  • Knee
  • Shoulder
  • Wrist

The following reasons for visit are documented on the dynamic HPI template using the non-laterality view for all HPI parameters:

  • Cervical spine
  • Jaw
  • Lumbar spine
  • Thoracic spine
  • Vestibular
