Associating a Patient Problem List Diagnosis to an ICD Billing Code for the Patient
You can associate a patient problem diagnosis list to ICD billing code for that patient. The Problem List panel grid on the Activate Care Guidelines template displays chronic condition and tracked condition diagnoses that are in the patient’s problem list. The problems are displayed with SNOMED® CT codes, which cannot be used for billing. If the diagnosis has not been previously billed, it will not have an ICD code for billing associated with it. To associate a diagnosis in the patient’s problem list with a care guideline, first you have to add the diagnosis to the ICD Billing list for the patient. After adding the diagnoses, you can return to the Active Guidelines panel to search and select the diagnosis with its ICD code to associate it to the guideline. Activating a Care Guideline
You can activate a care guideline for a patient on the Activate Care Guidelines template. You must activate a patient's care guideline for health maintenance or a disease condition before you use the care guidelines for the patient. You can activate guidelines in the Active Guidelines panel on the Activate Care Guidelines template. An application message appears if you want to re-apply an already activated guideline. You can re-apply a guideline if certain items within the guideline are not being followed and you need to add them now. Note: When you re-apply a guideline, the application checks through all of the guideline items that are already active, and applies those items in the set that are missing. If there is a test found in more than one guideline activated for the patient, only one test will be maintained. The application calculates the Due Date for that test based on the settings found for the earliest start age, the latest stop age, and the shortest interval of the activated

Restore a Guideline Item
You can restore a care guideline item for a patient using the Remove/Restore Guidelines panel. You can restore a care guideline or guideline item removed previously, so that it can be followed for a patient. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template. The Care Guidelines template opens. Select Add/Update in the Care Guidelines panel. The Activate Care Guidelines template opens. In the Activate Guidelines panel, select the Conditionfrom the list. Select Add to activate it again. This re-applies the guideline. Select Save & Close. Note: When you re-apply a guideline, the application goes through all of the existing items that are active, and only re-applies the one that is missing from the set for the condition. Reviewing Care Guidelines History
You can review the history of care guidelines tests on the Care Guidelines History template. A history of care guideline tests or exams can be reviewed on the Care Guidelines History template. You can see all guideline items that are completed and no longer due. For example, the peds guideline was applied to a 14 year old patient. Once the patient reaches adulthood, and all the well visits and other tests in the guideline are completed and no longer due, the peds guideline do not appear in the Care Guidelines template. Instead, these completed guideline items are seen on the Care Guidelines History template. Care Guidelines History The Care Guidelines History template can be opened from the: Care Guidelines panel on the Care Guidelines template. Active Guidelines panel on the Activate Care Guidelines template. This template opens from the Care Guidelines panel on the Care Guidelines template. Removing a Care Guideline or Individual Guideline Item
You can remove a care guideline for a patient in the Remove/Restore Guidelines panel. You can use the Remove/Restore Guidelines panel on the Activate Care Guideline template to remove individual guideline items or an entire care guideline by disease condition. You can also restore guidelines that are removed. You can do the following: Remove a guideline by condition is only available for the most current encounter for the patient. Remove an individual guideline item that is not followed while the remaining items for the guideline are followed for the patient. Remove a guideline in the current encounter, if you add a guideline by mistake or it no longer applies to the patient. If the guideline was added in a past encounter, historical documentation is saved, but cannot see the guideline in future encounters. You can activate a care guideline for a patient in the Activate Care Guideline template. The tasks that are done occasionally are performed on the Activate Care Guidelines template, such as: Activating a care guideline Removing a care guideline or guideline item Re-applying a removed clinical guideline or guideline item Viewing clinical references for guideline items Adding a diagnosis from the patient’s problem list to the patient's ICD billing list Reviewing care guideline history for the patient Note: You must activate a care guideline initially, usually only once. The guideline must be activated before it can be followed for a patient.
