Intake Visits for Behavioral Health

Intake visits are where the Comprehensive Assessment exam, including a substance use assessment and mental status exam are documented. Users can also choose to document an Individualized Action Plan (IAP), screening or admission note, or a psychiatric evaluation from an intake-based encounter. There are two types of Intake visits: with or without medical services. Intake No Medical Services can be used by anyone at the practice. Intake with Medical Services is designed to be used by providers who may be performing a physical exam and/or prescribing pharmaceuticals. Screening and Admission Visit Workflow

The BH - Screening/Admission Note template is used to document information that a program or Emergency Room might need for their initial evaluation of a client. Information gathered here is the basis for a summarization of the services and possible interventions that are initially provided to the client. Generally, this is completed by the non-clinical staff at the practice and can be done separately from a Comprehensive Assessment exam. The BH - Screening/Admission Note template can be accessed at any time during an encounter by selecting Other Templates from the bottom section of the navigation bar. Transfer or Discharge Visit Workflow

Set the Visit Type to Discharge Planning on the Home Page - Behavioral Health template to access the Transfer/Discharge tab, which includes three section tabs. The BH - IAP Review/Revision Template

The BH - IAP Revision/Review template is used to document progress or lack thereof and demonstrates evidence and/or rationale for the amendment of treatment goals and objectives in the Individual Action Plan (IAP). The system automatically pulls forward data from the last IAP or IAP review to the current encounter. BH - Individual Counsel/Psych Prog Note Template

The BH - Individual Counsel/Psych Prog Note template enables you to document individual counseling services and the client's progress or the lack thereof. Any risk assessment, goals, objectives, or assessment data that was entered previously on the Comprehensive Assessment template set populates in the panels found on the Progress Note tab. You can document new issues and make updates to the goals, objectives and interventions. Use the Behavioral health billing section to submit billable service codes to the Superbill. The Progress Note tab can be a stand-alone template or used in conjunction with the other tabs or links.
