General Details
On the Title X 2.0 template, the General panel displays a patient’s general information.
This table explains the fields in the General panel:
| Field | Description |
|---|---|
| First | The patient's first name is retrieved from the Patient Demographic template. |
| Last | The patient's last name is retrieved from the Patient Demographic template. |
| Middle | The patient's middle name is retrieved from the Patient Demographic template. |
| Encounter date | Displays the patient’s current or latest encounter date. |
| Family planning encounter | Select the Family planning encounter check box to include the current encounter details in the FPAR 2.0 report. |
| Contraceptive counseling | This finding is retrieved from the My Plan template. You can also select the check box to update. |
| Counseling achieve pregnancy | This finding is retrieved from the My Plan template. You can also select the check box to update. |
| Record contains substance use disorder information | Click the Information icon to understand the Record contains substance use disorder information check box. |
| Do you want to talk about contraception or pregnancy prevention during your visit today? | Select Yes or No option. |
| Encounter Insurance | For patient's who have encounter insurance, this field is automatically selected. |
| Self Pay | Self Pay is automatically selected when there is no insurance details. |
| No Charge Visit | When the No Charge Visit option is selected from the Finalize / Evaluation and Management Coding panel, No Charge Visit is selected automatically. |
