Enter Goals, Objectives and Interventions Addressed Today

Any goals and/or objective data that were previously entered on the Comprehensive Assessment template set populate in the panels found here. The grid on this panel offers a quick view of the client's current goals and objectives. In the Goals/Objectives Addressed As Per Individualized Action Plan section, field, click to Goal today select a previously entered item. Select an Objective today from the picklist of previously entered objectives. Click Add. In the Interventions Addressed Today section, do one or both of the following actions: Click Select From Previous IAP Intervention to select a previously entered value. Enter details in the Interventions/methods provided memo field. Click the My Phrases link to select a sentence to enter in the Interventions addressed today section. Click Add. Click Save & Close. In the Goals/Objectives Addressed As Per Individualized Action Plan section, Goal today field, click to select a previously entered item. Select the Objective today from the list previously entered objectives. Click Add. In the Interventions Addressed Today section, click Select From Previous IAP Intervention to choose a previous entered value. Enter the Interventions/methods provided. Record the Response to Interventions/progress toward goals and objectives. Document the Issues referred to physician/psychiatrist for consideration. Create a New Intervention

To enter a new intervention on the Intervention panel, click New Intervention. The BH - New Intervention template opens. Click the Linked to goal field to select a goal from the list and click OK. In the Linked to objective field, select an item from the list of previously entered objectives. Type an Intervention in the associated field. Click the Service description/modality field to select a value from the Modality list or to use the top, blank line in the list to type a different modality. In the Frequency field, do one of the following: Select a value from the list. Close the list to type a new kind of frequency. Set Responsible (type of provider) to a value available from the picklist or close the list to enter a different type of provider. Select Active or Inactive. Document Interventions, Methods, and Actions

Expand the Intervention(s)/Method(s)/Action(s) panel. In the Linked to goal field, click to select from the list of previously entered goals. Note: Linked to goal, Linked to Objective, and Intervention are required fields in this panel. In the Linked to objective field, select a previously selected objective. Enter the Intervention. In the Service description/modality field, select a type of therapy/group from the list or click on the top, blank line to type a value in the field. Set the Frequency of how often therapy and/or group sessions take place. In the Responsible field, select a type of provider from the Intervention Staff list or choose the top, blank line to free text a value. Select Active or Inactive. Click Add. Once the information has been added to the grid, additional interventions may be recorded as needed. When the IAP or BH Master document is generated, the intervention, method and/or action information appears under the status selections made on the BH - Individualize

Add a New Goal for IAP Reviews

When entering data in the Goals and Objectives Status Section panel, it is important to note that needs and interventions are not mandatory to complete the plan, but goals are required. The grid at the bottom of this panel offers a quick view of the client’s current needs, goals, and objectives. In the Goals and Objectives Status Section panel, select the New Goal link to open the BH- New Goal template. BH-New Goal Select a previously entered need for Linked to assessed need and click OK. In the Functional area field, select from the list. Type a new Goal and enter the Start date and Target completion date. Note: If the start and target dates are not entered before selecting an Adjusted target date, the system alerts the user to enter them. Enter the Desired outcomes for assessed need in individual's words. Record the Supports, resources, organizations and individuals needed to meet this goal. Answer No, Yes, or Yes, but deferred to the question: Has a problem been identified and enter

In the Goals/Objectives Addressed as Per Individualized Action Plan section, click the Goal today field to select an item. Select the Objective today from the list. Click Add. Select the appropriate Stage of treatment. In the Therapeutic Interventions Delivered in Session section, select any option that applies under Describe interventions. Enter information in the Comment on all interventions selected field. Under Counseling provided with client/family/caregiver, select from the following topics: Diagnostic results/impressions and/or recommended studies Instruction for management/treatment and/or follow-up Importance of compliance with chosen treatment Client/family/caregiver education Risks and benefits of treatment options Prognosis Risk factor reduction Enter details for each selection in the Comments field.
