Edit Assessments
You can edit assessments on the Assessment and Plan template.
- In the Status grid, click Edit an Assessment.
The Assessment grid displays the assessments for the current encounter.

You can copy historical diagnosis, assessment, and plan using the Copy Historical DAP template. Open NextGen® Enterprise EHR. Open the Soap - Cardiology template. Click the Assessment/Plan panel. Click Copy Forward. The Copy Historical DAP template opens with the All DAP and the Diagnosis/Assessment/Plan options selected. Copy Historical DAP - All DAP Note: To prevent duplicate entries, diagnosis codes from the current encounter do not appear in the DAP History grid. Plans with orders do not appear in the grid and cannot be copied to the current encounter. Select the D (diagnosis) row associated with the DAP. The Copy Dx/AP button is enabled. Click Copy Dx/AP. CAUTION: It is possible to copy an expired ICD code from a previous encounter to the current encounter. Clients must review copied ICD codes and change the code to a valid ICD code, when applicable. Open NextGen® Enterprise EHR. Open the SOAP template. Click the Assessment/Plan panel. From the left pane, click an assessment category link. The diagnosis search window displays a list of diagnoses for the category selected. Select an appropriate diagnosis and then click Select. The diagnosis has been added to and displayed in the grid with the letter “D” and lines for A and P. Assessment/Plan Panel Click the A row in the grid for a diagnosis. The Assessment And Plan template opens. Assessment And Plan Template Note: Certain diagnosis requires you to choose a category before documenting the details. This allows the appropriate assessments that match the ICD-10 code to display. To enter an assessment or plan, do one of the following: Click Type an assessment below the Status field. Click Type a Plan below the Plan Type field. Note: To use phrases, click the My Phrases link and then select an option from the list. You can copy only the historical diagnoses to the current encounter on the Copy Historical DAP template. The Diagnosis Only option enables you to view and copy previous three years diagnosis details from encounters to the current encounter. Open NextGen® Enterprise EHR. On a patient encounter, open the Soap - Cardiology template. Click the Assessment/Plan panel. Click Copy Forward. The Copy Historical DAP template opens with the All DAP viewing option selected. Select the Diagnosis Only option. Copy Historical DAP - Diagnosis Only Note: To prevent duplicate entries, diagnosis codes for the current encounter do not appear in the DAP History grid. The Copy Historical DAP template displays the patient's historical diagnosis. Select a diagnosis. The Copy Dx/AP button is enabled. Click Copy Dx/AP. You can enter assessment and plan details on the Add Assessment tabbed template. Open NextGen® Enterprise EHR. From the SOAP template, in the Assessment / Plan panel, select Add/Update. The Add or Update Assessment tabbed template set opens. Select an assessment. Enter the Status, Side, Site, Impression, and Differential dx fields. Select Add/Update. Open NextGen® Enterprise EHR. In the Assessment / Plan panel, select an assessment in the grid. The Add or Update Assessment template opens. In the Today's Assessment grid, select an assessment.
