Add Substance Use Data
The data recorded for the Alcohol and Drug Use/Abuse sections at the top of the Substance Use panel carry forward to NextGen® Adaptive Content Engine core templates and vice versa. If details have been entered for drug or alcohol use on the core NextGen® Adaptive Content Engine templates, that data populates under Primary, Secondary, etc. For privacy reasons, information entered on the Behavioral Health, Substance Use Assessment template does not carry forward to the core templates that hold this type of information. On the BH Substance Use Assessment template, if appropriate, place a check mark next to No addictive behavior reported on the panel header. Otherwise, expand the Substance Use panel. Under the Alcohol section, select No, Yes, or Formerly for Drinks alcohol. Enter an answer in the How many days in the past year have you had 5 or more drinks in a day field. Under Drug Use/Abuse, select No, Yes, or Formerly for Uses drugs. Enter the Primary, Secondary, Tertiary drugs. To ente

Enter Toxicology Screen Data
On the BH Substance Use Assessment template, expand the Toxicology Screen panel. Click Yes or No for Toxicology screen completed. To enter detailed results, click the Results, if available field. Enter Chemical Use/Abuse History
On the BH Substance Use Assessment, expand the Chemical Use/Abuse History panel. Click the Describe progression of use/relapse history field to free text or use voice recognition software to detail the client's historical information, including any attempts to abstain from drug use, reason for relapse, prior periods of sustained recovery and how that recovery period was sustained. Enter the Number of times stopped. Enter the Longest period of abstinence. Click the Tolerance/potential for withdrawal field to type related information. Substance Use Assessment Exam
BH Substance Use Assessment is a template on which a clinician or nurse can gather information about a client’s substance use history and how an addiction might negatively impact the client’s life. The information gathered on this template is configured to appear on whatever document is generated. It is one of just a few templates that launches with all but the top panel collapsed. This was done to make it easier find and use the appropriate panel. The recommended workflow is to expand a panel to document the information and collapse it when that section is complete. This template is launched from a Substance use link found on the Substance Use/Addictive Behavior panel. This link is also available on many templates throughout the Behavioral Health template set. Document Current Tobacco Use
Expand the Tobacco Use panel. Select the appropriate option, if the patient: Currently uses tobacco, select Yes for to Have you ever used tobacco. Smoked tobacco in the last 30 days, select Yes for to Have you used tobacco in the last 30 days, and then record the types, quantities, and frequencies under the Smoking Tobacco Use section. Chews tobacco or takes snuff, select the Yes option for Have you used smokeless tobacco in the last 30 days, and then record the types, quantities, and frequencies under the Non-Smoking Tobacco Use section. Click OK or expand other tobacco template panels as needed to document additional tobacco history or select another social history from the side navigation.
