Enter Assessment Plan Details

Open NextGen® Enterprise EHR. On the Assessment Plan Details template, select the A/P Details tab. To add or change a diagnosis in the Today’s Assessments grid, select Add or Edit to open the Add or Update Assessment template. To change the current working assessment, in the Today's Assessments grid, select the diagnosis being addressed for this encounter. The selected diagnosis appears in the Selected Dx field. It becomes the new working assessment that carries forward to the other templates that share the assessment. Enter details manually or use My Phrases or Common Phrases for the impression, differential diagnosis, and patient and provider plans in the respective sections. To enter follow-up appointment and disposition information, select Follow Up. To document counseling educational details and factors, select the Counseling Details link. Document Office Procedures

Open the ASC - Physician Procedure template. In the Assessment / Plan panel, click the Office Procedures link. The Office Procedures template opens. If required, select an assessment. The Office Procedures template is part of a group of templates that use the last assessment or diagnosis selected on any of the templates in the group as the working assessment. The application defaults to the current working assessment. You can select another assessment, which becomes the new working assessment that carries forward to the other templates in the group. The selected assessment appears in the Selected Dx field. Select an appropriate procedure and enter the required details. Office Procedures Click Place Order. Order Therapies in the Therapy Order Template

You can place orders for various therapies for a patient and generate the order requisition. Open the Home or any other applicable template, select Orders from the upper navigation bar, and then select Therapy Order. The Therapy Order template opens. Note: The Date field displays the current date. You can update the date if needed. To add the diagnoses and procedures, select Dx & Px. Note: Selecting procedures is optional; however, you must select at least one diagnosis to order therapies. After you place the order, the documented diagnoses and procedures appear in the grid as follows: If you add more than four diagnoses, the first four diagnoses appear in the Diagnosis column, and the remaining appear in the Summary column. If you add more than one procedure, the first procedure appears in the Primary Sx column, and the remaining appear in the Summary column. Copy Assessments from Previous Encounters

You can copy assessments from previous encounters of a patient to the current encounter instead of manually adding them. OpenNextGen® Enterprise EHR. Open theSOAPtemplate, and then expand theAssessment/Planpanel. Select Dx/AP History. The Assessment, Diagnosis Copy Forward window opens. Note: By default, the All Assessments option is selected and records are displayed for the default duration configured in the Copy Previous Visit Information section of the Practice Configuration template. To copy assessments from a specific encounter, select Encounter Assessments. To filter the records in the grid, enter the search criteria inFilter by, and then selectSearch. Note: If the records are displayed from any sensitive encounter, an asterisk (*) appears on the record in theSensitivecolumn to help you decide whether to copy such records and update the current encounter as sensitive. Document a Therapy Referral

You can select the Medications or Immunizations links at any time during the encounter to access the associated module. Open NextGen® Enterprise EHR. To select the insurance provider from the resulting list, on the Referrals tab, select the Insurance name field. In the Policy # field, enter the insurance policy number. In the To section, select Therapies/Rehabilitation, and do one of the following: Select a value from the resulting list. Select the top, blank row in the list to enter a therapy name in the field. Select the Location field and do one of the following: Select a value from the list. Select the top blank row to enter a location in the field.
