Document the IUD Removal Details

You can document the Intrauterine Device (IUD) removal procedure details in the IUD Removal section on the IUD - Procedure template. Open the IUD - Procedure template. In the IUD Removal section, select a diagnosis in Indication for procedure. IUD Removal section If you select the Indication for procedure check box, the procedure and its code are automatically added in the respective fields. Select a status for the diagnosis. Enter the reason for removal, side effects, and the desired birth control method. In Strings visible, select No or Yes. Enter the instrument with which the Intrauterine Device (IUD) is removed. Optional: Select the IUD removed without complications and Patient desires reinsertion check boxes. Document the IUD Insertion Details

You can document the Intrauterine Device (IUD) insertion procedure details in the IUD Insertion section on the IUD - Procedure template. Open the IUD - Procedure template. In the IUD Insertion section, select a diagnosis in Indication for procedure. IUD Insertion section If you select the Indication for procedure check box, the procedure and its code are automatically added in the respective fields. Select a status for the diagnosis. Optional: Select the History reviewed and no contraindications check box. Enter the uterine size and uterus details. Enter the name of the chemical used in cleaning and preparing the cervix. Document the Post-Procedure Details for IUD

You can document the post-procedure care details of the Intrauterine Device (IUD) in the Post-Procedure section on the IUD - Procedure template. Open the IUD - Procedure template. In the Post-Procedure section, select the following check boxes as applicable. Patient tolerated procedure well Patient to follow up after next period Patient instructions given Post-Procedure section Enter your comments. Optional: To document the medication details, select the Medication Module link and then enter the details in the Medication module. You can enter the Intrauterine Device (IUD) details in the Medication module with their expiration dates. To document assessment and plan details, select the A/P Details link, and enter the details on the Assessment Plan Details template. Select the procedures and add the necessary modifiers. Select the number of days or weeks for the follow-up checkup or the date. Note: If you select the time interval for the next checkup, date automatically appears in Date. O

You can document the pre-procedure care details of contraceptive implant in the Pre-Procedure Care section on the Contraceptive Implant Procedure template. Open the Contraceptive Implant Procedure template. In the Pre-Procedure Care section, select LMP. If LMP is entered on the Vital Signs template, LMP appears automatically in the LMP field. Optional: Select the Postpartum check box. Enter the pre-op contraception details. Optional: Select the following check boxes as applicable.
