Document the patient's medical and infection history and you can update when required.

If the patient provides any positive answers to any of the medical or infection questions, then enter the details in the
Medical/Surgical/Interim
panel.

Document the Medical/Surgical/Interim Panel
You can filter the past history records in the grid by selecting the Medical History/Surgical History/Interim History check boxes. By default, Show All check box is selected. Open the Histories tab. Open the Medical/Surgical/Interim panel. Select Add to open the Past Medical History template. Select the check box corresponding to the correct entry. Note: Items in blue are categories rather than individual selections. Selecting the blue name, or selecting the check box opens a list of choices. The Add Pregnancy Detail button in the Medical panel opens the Pregnancy Outcomes template. Select the Add to Grid button. Past Medical History Expand Surgical to enter past procedures/operations. Surgical panel on the Past Medical History template Select Save & Close. A new past medical history record displays in the panel on the Histories template. Note: If information is entered in the Medical/Surgical/Interim grid during prior visits or by other specialties, that information also displays. Doc

Document the OB Histories Template
To document information in the OB Histories template, select the Histories tab. By default, all panels on this template are expanded when the template is opened. Document the Pregnancy History Panel You can document pregnancy history information in the Pregnancy History panel. Document the Gravidity/Parity Panel You can quickly view the patient's gravidity and parity and can only document multiple births information in the Gravidity/Parity panel. Document the Since Last Menstrual Period Panel You can document last menstrual period information in the Since Last Menstrual Period panel. Document the Medical/Infection History Panel You can document the medical infection history information in the Medical/Infection History panel. Document the Interim History
From the Histories template, expand the Medical/Surgical/Interim panel and then click Interim History. The Interim History window opens. Select an encounter type. If an encounter type does not appear in the list, you can manually enter it. Click the Problem field. If problems are added during previous encounters, the Patient Problems template opens. If there are no problems from previous encounters, then a search window opens. Do one of the following: From the patient problems list, select an existing problem. From the search window, search for and select a new problem and then click the Add to Problem List button. Note: If you manually enter the problem, the Add to Problem List button is not enabled. In the Management field, enter the therapy or procedure performed on the patient. Select the current date and then the provider who performed the therapy or procedure. If a provider does not appear in the list, you can manually enter their name. Enter admission details, discharge details,

Update Past Medical History
You can update the past medical history for a patient on the Edit Past Medical History template. Open NextGen® Enterprise EHR. Do one of the following to update past medical history on the Edit Past Medical History template: On the Histories template, in Medical/Surgical/Interim, select the corresponding medical or surgical record from the grid that you want to update, and select Edit. In the History Summary panel on Intake, in Past Medical/Surgical, select the row on the grid that you want to update, and select Edit. The Edit Past Medical History template opens. Edit Past Medical History Note: The Edit Past Medical History template can also be accessed from the Past Medical History template by selecting an entry on the Past Medical History grid and selecting Edit.
