General Dermatology Workflow

The general dermatology workflow offers a broad framework for conducting a dermatological visit. The workflow provides guidelines for conducting a comprehensive patient encounter using the Dermatology specialty of NextGen® Adaptive Content Engine. You can modify this workflow to meet your own specific needs. In addition, your office staff can be organized differently and their duties may be defined differently. Regardless of who executes each steps, the functions remain the same. The general office and virtual visit workflow includes: Intake staff workflow Provider workflow Checkout staff workflow Note: The instructions provided for office visit are also applicable for virtual visits. Intake Staff Workflow The intake staff can use the Intake template to document a patient's chief complaint, vital signs, medications, allergies, standing orders, and various other findings. Documenting Patient Encounters for Dermatology Visits

The Dermatology User Guide forNextGen®Adaptive Content Engine provides the guidelines for conducting a comprehensive patient encounter in the Dermatology specialty. During Dermatology patient encounters, you can use various features that are designed to streamline your workflows and improve your data captures. The intake staff checks in the patient on the Intake and Histories templates. If required, the intake staff can review the patient's details documented in the previous encounters on the Home Page template. The provider can choose to review the patient's historical information documented in the previous encounters on the Home Page template or review the Intake Note document generated by the intake staff. After reviewing the information, the provider documents the patient examination on the GET and SOAP templates. The provider completes the visit and selects the E&M code on the Finalize template. Mohs Surgery

Mohs surgery can only be performed on pathology proven lesions, that is, the patient must have pathology proven skin cancer items in the Skin Cancer History Log. This information is added during the office encounter visit when a biopsy is ordered and during the subsequent chart history encounter when the pathology results are documented and added to Skin Cancer History Log. General Workflow for Mohs Surgery The Mohs surgery process workflow includes biopsy encounter, chart encounter, and Mohs surgery encounter. Record the Intake for Mohs Surgery You can document the intake workflow for a Mohs surgery. Access and Document Mohs Surgery Procedure The provider workflow process for Mohs surgery is from pre-procedure to submit procedure is detailed. Document a Routine Visit

After the intake staff completes their work with the patient, you can document the patient encounter. On Dermatology Home Page template, review the patient information entered by the intake staff. On the Histories template, review the patient information and make the desired modifications. On the SOAP template, view the details of reason for entered by the intake staff in the Reason for Visit panel. You can enter additional reasons for visit. In the Review of Systems panel, to document patient's signs and symptoms, do one of the following: Click the ROS defaults field, and then select an appropriate default ROS from the list. Click DERM - ROS and do one of the following in the Dermatology-ROS template. To select negative or positive for symptoms in a system, select either Neg or Pos options. To mark negative for all symptoms in a specific system, select All neg in the respective system. To document the findings in detail for a specific system, click the respective system link, and then

Access Dermatology

Open NextGen® Enterprise EHR. Select a patient or create a patient. Create a patient encounter. To open the Templates module on a patient encounter, do one of the following: On the Categories or Encounter History tab, select Open Template. On Top Toolbar, select Templates . On the main menu, select File > New > Template. The Select Template window opens. Select Home Page - Dermatology, and then click OK.The Home Page Dermatology template opens. To review the page, select the Visit Type. From the navigation bar at the top of the Home Page - Dermatology template, you can access the Dermatology framework templates like Intake, Histories, GET, SOAP, Finalize, and Checkout. Provider Workflow

The provider workflow includes the following: Reviewing and updating the data documented by the intake staff during the patient encounter and providing any additional information. Documenting the problems and various physical examination findings. Documenting the performed procedures. Documenting the treatment plan. Documenting Evaluation and Management (E&M) codes. Review of Dermatology Details by Providers You can start the visit by reviewing the patient information documented during the previous encounters. Dermatology Home Page Template You can review the historical information of the patient on the Home Page - Dermatology template. Overview of the GET Template You can use GET template instead of SOAP. Documenting SOAP Information You can review and modify the patient’s reasons for visit, review of systems, and vital signs documented by the intake staff on the SOAP template.
