In the NextGen® Adaptive Content Engine templates, the following types of History of Present Illness (HPI) templates are used to document a patient's reason for visiting the office: Static HPI - displays hard-coded content that is not easily customizable; a single template is available for each complaint with options for selecting pre-determined details: the final concatenation is available when you close the template Dynamic HPI - enables you to add, edit, update, and delete dynamic content for the HPI and concatenates the content on the template as items are selected or edited; NextGen® Adaptive Content Engine is packaged with pre-configured dynamic HPI templates for the Gastroenterology, Physical Therapy, Orthopedics, Urology, and Pediatrics Well Child; the specific details about these existing HPI elements are provided in the Specialty Specific Dynamic HPI Elements section of this user guide. The dynamic HPI templates for the Pediatrics Well Child visit type have both generic and distinct sections available for documentation. Generic sections are common to all reasons for visit and distinct sections are specific to each reason for visit. Combinations of these sections enable the complete documentation of the patient's reason for visiting the office during the current encounter. The following table defines the distinct and generic sections that appear for all Pediatrics, well-child reasons for a visit. Parameter Description All Normals Enables you to document or edit additional details or abnormal findings. The Well Child visit in Pediatrics enables you to document age-specific complaints for the following reasons for visit: Well Child birth to 1 Year Well Child 1-4 Years Well Child 5-10 Years Well Child 11-21 Years Note: This topic provides information on only those reasons for visit that opens dynamic HPI template associated with Pediatrics, Well Child visit. Treatment visits can be based on the non-laterality view as most patients do not have an issue that pertains to one side of the body or the other. You can dynamically document the reason for visit of a well child using the Well child HPI in the Reason for Visit panel. If the intake staff selects the Well child HPI reason during the intake process, the Well child HPI reason automatically appears on the Reason for Visit panel. If the intake staff does not select the Well child HPI reason, you can select the Well child HPI reason from the left pane. Open the Pediatric SOAP template. In the Reason for Visit panel, click Well child HPI. The HPI template opens. HPI Note: By default, the application selects the well child age group in the Reason field based on the patient's date of birth. To change the well child age group, click the list in the Reason field and select a different well child age group. Document the age-specific complaints about the well child visit. Open the Intake template. Select the appropriate specialty name from the Specialty list. For example, select Gastroenterology. Select Office Visit from the Visit Type list. Click the SOAP tab. Do one of the following in the Reason for Visit panel: Click an existing reason for visit in the grid. Select a new reason for visit. Select vomiting. Figure 1. . You can use the Reason for Visit panel on the Pediatric SOAP template to review and update the reasons for visit added during the intake process. The Well child hpi appears on the grid automatically if you select Well child for Visit Type. Reason for Visit In addition to documenting the specialty specific reason for visit available in the left pane, you can also document a well child visit on the a generic HPI template by selecting Well child or Well child HPI in the Reason for Visit panel. In the Reason for Visit panel, you can: Review and update the social history of the patient using the Pediatric Social History link. Open the Interval History- Nutrition template using the Nutrition/Elimination link. Document substance use, psychiatric, placement, sexual practices, and child abuse history of the patient using the Confidential History link.
