You can record the echocardiogram test details and create a test report. The Echocardiogram template consists of the following panels. The intake staff documents the Procedure Information which includes pre-procedure care, test/patient data, vital signs, and technical details. Findings Summary Note: The fields marked with asterisk * are required. Document Echocardiogram Procedures You can document the Echocardiogram procedure from the Echocardiogram template on the Echo navigational link. Echocardiogram Test Findings You can document the Echocardiogram test findings on the Echocardiogram template on the Echo navigational link. Document Summary of Echocardiogram Tests You can document a summary of the Echocardiogram test in the Summary panel. Signing off on Echocardiogram Test Findings You can sign off the Echocardiogram test when all the required fields have been entered in the Echocardiogram template. Document Holter Procedures

You can record the Holter Monitor test details. The Intake staff documents the procedure information which includes pre-procedure care, test/patient data, vital signs, stress data, recovery data, and wall motion. Open NextGen® Enterprise EHR. Click the Intake tab. Select Cardiology as Specialty and Testing Only as Visit Type. Click the Holter link. Note: You can also access Holter Monitor template from the left Navigation bar. The Holter Monitor template opens. Holter Monitor-Procedure Information Click the Procedure Information panel. Click the Indication field and select an indication. Click the Hook Up Date field and select the date when the holter monitor has been hooked. Click the Disconnect Date field and select the date when the holter monitor has been disconnected. Click the Send Copy To field and select a physician to whom you want to send a copy of the test report. Click Ordering Dr and select the ordering physician. Click Diary and select an option. Test or Patient Data Section for Navigational Links

There are multiple fields in the Test/Patient Data section for the cardiology navigational links. Field Description Test date Displays the current encounter date by default. Test type Displays Internal by default. Location Displays the encounter location by default. Reading This field is populated when the test is signed by the reading physician. Echo type Displays the echo type. Referring 1 Lists the referring providers. The Telephone icon displays the referring provider's telephone and fax information. This field retrieves the name of the provider if the echo test has been performed in a previous encounter. Referring 2 Used to select another referring provider if required. The Telephone icon displays the referring provider's telephone and fax information. Archive Used to enter an archive name to record the corresponding tape number. Document Stress Echo Procedures

You can document the stress echo procedures using the Stress Echo template. Open NextGen® Enterprise EHR. On the Intake tab, select Cardiology as Specialty and Testing Only as Visit Type. Click the Stress Echo link. Note: To access the Stress Test - Card template, you can also click Stress Echo from the left navigation bar. The Stress Test - Card template opens. Select the desired options in the Pre-procedure care section. Procedure Information on Stress Test - Card template Enter the desired information in the Test/Patient data section. In the Vital Signs panel, the values for the following fields display automatically. Predicted max heart rate is calculated based on the patient’s age. Predicted max heart rate 85% is calculated based on the patient’s predicted maximum heart rate. Predicted max O2 consumption is calculated based on the patient’s age and gender. Achieved VO2 consumption is calculated based on exercise duration. Percent VO2 achieved is calculated based on Achieved O2 con

Document the Findings of Stress Echo Tests

The rendering provider can document the findings of the Stress Echo test in the Findings section. Open NextGen® Enterprise EHR. On the Intake tab, select Cardiology as Specialty and Testing Only as Visit Type. Click the Stress Echo link to access the Stress Test - Card template. Note: To access the Stress Test - Card template, you can also click Stress Echo from the left Navigation bar. Enter Exercise Duration. Note: The values for MHR, PMHR, and DP (SBPxMHR) display based on previously documented stress data. The value for NYHA functional class and METS appears. Enter Pharmacologic Agent Maximum Dose. Select Yes or No options for Target Heart Rate achieved and Maximum Heart Rate achieved. Click the Stopping Reasons field and select one or more options from the list. Cardiology Testing Navigational Links

You can find the cardiology specific testing navigational links when you select Testing Only as the visit type. You can use these links to document the tests and results.
