Screening Tools and Follow-Up Plans of Care

You can document the details of the screening tools and follow-up plan of care for a patient.

You can document the details of an assessment, a template, or website screening tool, and calculate the associated score using the Screening Tools link.

If an assessment is not available, you can select a tool in the Screening instrument section and document the assessment. The documented data appears in the Screening Tools Performed grid.

To document the follow-up plan of care in the Functional Status panel, you can send a notification to the required recipients using the Task button.
