Managing Patient Care Guidelines

Using a work list, you can view, manage, plan, and track a patient's care guidelines and other administrative tasks and complete them on time. When you apply filters on the Pre Visit Planning template, it does not impact the Work List panel. You can use the Care Guidelines template to verify or update the information related to clinical guidelines, screening questions, metrics, vital signs, immunizations, nutrition, drug therapy, and recent orders. The Care Guidelines section includes a view-only grid which contains guideline items documented on the Care Guidelines template. You can use the Filter field to view information based on the patient's conditions. The filter selections only affect the information in the grid within the Care Guidelines section. Pre Visit Planning

You can use the Pre Visit Planning template to review a patient's chart and take appropriate steps to be prepared for an upcoming patient visit. Pre Visit Planning The Pre Visit Planning template enables clinical staff to handle administrative tasks before a patient's appointment. Managing Patient Care Guidelines You can manage the patient care guidelines and track the administrative tasks on the Work List panel. Reviewing Assessment and Plan History You can review the assessment and plan details of a patient during a visit. Bookmarking Documents and Images You can bookmark a patient's documents and images for quick reference. Viewing Communication Details On the Pre Visit Planning template, you can view a patient's communication details like phone calls, appointments, and outstanding tasks. Add a Problem in the Problem List

You can add a problem to the problem list of a patient. Open the Histories template. In the Problem List panel, click Add/Update.The Problems template opens. Click Add Problem.A search window open. Enter the full or partial name of the problem in the field at the top of the search window. Click Search.A list of search records displays under the Description column. Select a problem and click Select. Note: To add the selected problem to the tracked problem list, select the Add to My Tracked Problems check box. The selected record displays in the Problem List module. From the Problem Status list, select the status of the problem. If the selected problem is chronic, select the Chronic check box, otherwise select the Secondary Condition check box. Note: The Onset Date field displays the current date. You can enter an approximate date if the exact onset date is unknown. In the Resolved Date field, enter the date when the selected problem was resolved. Click Accept. The problem is grouped acc

Update a Patient's Problem List

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. Open NextGen® Enterprise EHR. On the Care Transitions tab, and expand the Problem List after Discharge panel. Select the appropriate checkbox to build the description of services required for the patient. If required, type additional text into the description. If the patient currently does not have any active problems, select the No active problems option. To add a new problem to the list, select Add.The Problem module appears. Note: For information on using the Problem module, go to the NextGen Healthcare Success Community, and download the Problems Module User Guide for NextGen® Enterprise EHR. Open Problem List

You can open the problem list to document and view a patient's problem history. Open NextGen® Enterprise EHR. Select a patient record. To open the Problem List panel, do one of the following: In the Histories template, expand the Problem List panel. In the Home Page template, expand the Problem List panel. Record, modify, and retrieve a patient's clinical problem list electronically to meet Quality Reporting requirements Document the patient's chronic conditions and ongoing clinical problems View problems from past encounters Add problem details Problem List You can also indicate if a patient's documented problem is completed and reviewed, and if there is any active problems.
