Vital Signs Grid

Select the Height and weight checkbox. Enter the patient's height in the Ht(ft) and Ht(in) fields. The system calculates the patient's height in centimeter and displays in the Ht(cm) field. If you document the patient's height in the Ht(cm) field, the system calculates the patient's height in feet and inches and displays in the Ht(ft) and Ht(in) fields respectively. Enter the Vital Signs Monitoring Details

Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the ASC-Intake template. In the Vital Signs Monitoring panel, select Add. To indicate the baseline set of vitals, select the Baseline checkbox. . To enter the time when the vitals were taken, in Time, do one of the following: Select the Time field arrow and enter the time on the Time Entry template. Select to enter the current system time in the Time field. Note: The asterisk (*) indicates that the Time field is mandatory. Enter the measured vitals in the following fields: Temp within the 50 – 121.98 F range, Pulse within the 10 – 499 range, Resp within the 0 – 99 range, Sys BP and Dias BP within the 0 – 399 range, SpO2 within 0 – 100 range, CO2, ECG rate, and ECG rhythm. Record the Pre-Procedure Vital Signs of the Patient

In the Vital Signs Monitoring panel, select Add.The Vital Sign Monitoring template opens. The Baseline Vital Signs section includes a grid to display only the baseline set of vitals documented in the Vital Signs Monitoring section in the template. In the Vital Signs Monitoring section, enter the required details. Select Save & Close to display the documented vital signs in the grid.The baseline vitals display at the top row of the grid throughout the workflow. The Height, Weight, BMI, and BSA above the grid displays the height, weight, and the calculated BMI and BSA values from the Vital Sign Monitoring template. The values associated with the Height, Weight, BMI and BSA labels above the grid dynamically update as you modify or add height and weight data on the Vital Sign Monitoring template. Document Vital Signs

You can document vital signs for the patient on the Vital Signs Lite template. Note: To document numeric values for height, weight, temperature, pulse, and respiration quickly, enter the values using the number pad. Click Next to move to the next field. Click Add below the Vital Signs grid. The Vital Signs Lite template opens. To document unobtainable vital signs, select one of the following check boxes, and then select the vital signs that were not obtained from the list. Unobtainable Patient Refused By default, the Measured date and Time fields display the current system date and time. Click the Time field to select a value to enter vitals measured at an earlier time.By default, the current user information appears in the Measured by field and cannot be changed. Vital Signs

You can add, edit or remove vital signs for a patient using the Vital Signs panel. In the Vital Signs panel you can view vital signs documented during this encounter. You can edit ot remove previously entered vital signs and enter new vital signs. The Vital Signs panel is also available on other office visit templates. The Historical information entered this encounter link opens the Vital Signs History template filtered to display only vital signs documented with patient's encounter. Using the History link, you can review the patient’s entire vital signs history on the Vital Signs History template. Using the Graphs link, you can view vital signs in graphical format on the age-appropriate VS Charts template. Vital Signs The vital signs templates are based on age. The age-appropriate template opens depending on whether the patient is less than two years old, equal to or greater than two years old, or an adult. Patient Vital Signs

You can document and manage a patient's vital signs. You can add various vital signs for a patient during an encounter. The most recent value for each vital sign appears in the grid. A number appears adjacent to the vital sign to indicate the number of times a value is recorded for the vital sign. You can point to the number to view the values and the time when each value is recorded. Vital Signs Section You can view only the vital sign records documented for the current day in the grid. To view the historical vital signs records, you can use the History link. To view the vital signs graphs you can turn on the Graph link. If the vital sign interface is installed, you can use the Import and Refresh buttons to open the VitalSignsSpotMonitor interface. Based on the configuration done in the Vital Signs section on the Practice Configuration template, you can turn on the Import and Refresh buttons and manage the security groups for the alerts.
