You can open the physical exam template to document the normal and abnormal physical exam details.
Open the
SOAP
template.
In the
Physical Exam
panel, select a specialty-specific physical exam in the left pane.
The physical exam template opens.
Note:
If your desired physical exam is not available in the left pane, click
Additional
and select the physical exam.

Document Normal Findings for the Shoulder
Open the SOAP template. In the Physical Exam panel, click Shoulder in the left pane.The Shoulder - PE template opens. Shoulder - PE In the Quick exams field, select the appropriate checkboxes and click the corresponding field to display the list. If appropriate, select Normal from the list.The respective fields in the Shoulder - PE template populate with the normal values. Enter diagnosis codes in Dx1 or Dx2. Open the SOAP template. In the Physical Exam panel, click Wrist in the left pane.The Wrist - PE template opens. Wrist - PE In the Quick Exams field, select the appropriate checkboxes and click the corresponding field to display the list. If appropriate, select Normal from the list.The respective fields populate with the normal values. Enter diagnosis codes in the Dx1 or Dx2 fields. Open the SOAP template. In the Physical Exam panel, click Hand in the left pane. The right and left options appear in the sub-menu. Click right to open the Hand Right - PE template or click left to open the Hand Left - PE template. Hand Right - PE If appropriate, click the Quick Exams field and select Normal from the list. The respective fields in the Hand Right - PE template are filled with the normal values. Select the diagnosis codes from the Dx1 or Dx2 fields. Document a Multi System Orthopedic Exam
Open the SOAP template. In the Physical Exam panel, click One Page Exam in the left pane.The General Exam - PE template opens. In Location, click the region of injury from the Ort Regions list. In Quick exams, select the appropriate checkboxes and click the corresponding field to display the Td Dbp Filter list. Select Normal from the Td Dbp Filter list.The respective fields in the General Exam - PE template are filled with the normal values. Select the diagnosis codes in the Dx1 or Dx2 fields. Click the corresponding Status field to select the status of each diagnosis. Click OK.
